Evaluation of percentage of free serum prostate-specific antigen to improve specificity of prostate cancer screening.

OBJECTIVE To evaluate measurement of percentage of free prostate-specific antigen (PSA) in serum to improve the specificity of prostate cancer screening in men with serum PSA levels between 4.1 and 10.0 ng/mL. DESIGN Retrospective, nonrandomized analysis using a research assay for measuring free PSA in frozen serum from men with a spectrum of prostate sizes and digital rectal examination results. SETTING General community outpatient prostate cancer screening program at a university center. PATIENTS One hundred thirteen men aged 50 years or older, 99% of whom were white, with serum PSA concentrations of 4.1 to 10.0 ng/mL, including 63 men with histologically confirmed benign prostatic hyperplasia, 30 with prostate cancer with an enlarged gland, and 20 with cancer with a normal-sized gland. All study volunteers had undergone prostatic ultrasonography and biopsy. MAIN OUTCOME MEASURES Percentage of free PSA in serum and percentage of free PSA cutoff that maintained at least 90% sensitivity for prostate cancer detection. RESULTS Median percentage of free PSA was 9.2% in men with cancer and a normal-sized gland, 15.9% in men with cancer and an enlarged gland, and 18.8% in men with benign prostatic hyperplasia (P < .001). The percentage of free PSA cutoff was higher in men with an enlarged gland and in those with a palpably benign gland. In men with an enlarged, palpably benign gland, a free PSA cutoff of 23.4% or lower detected at least 90% of cancers and would have eliminated 31.3% of negative biopsies. CONCLUSIONS Measurement of percentage of free serum PSA improves specificity of prostate cancer screening in selected men with elevated total serum PSA levels and can reduce unnecessary prostate biopsies with minimal effects on the cancer detection rate; however, further studies are needed to define optimal cutoffs. Final evaluation of PSA screening also must consider the ability of current treatments to improve the prognosis of screen-detected prostate cancer.

AI and Worker Well-Being: Differential Impacts Across Generational Cohorts and Genders

This paper investigates the relationship between AI use and worker well-being outcomes such as mental health, job enjoyment, and physical health and safety, using microdata from the OECD AI Surveys across seven countries. The results reveal that AI users are significantly more likely to report improvements across all three outcomes, with effects ranging from 8.9% to 21.3%. However, these benefits vary by generation and gender. Generation Y (1981-1996) shows the strongest gains across all dimensions, while Generation X (1965-1980) reports moderate improvements in mental health and job enjoyment. In contrast, Generation Z (1997-2012) benefits only in job enjoyment. As digital natives already familiar with technology, Gen Z workers may not receive additional gains in mental or physical health from AI, though they still experience increased enjoyment from using it. Baby Boomers (born before 1965) experience limited benefits, as they may not find these tools as engaging or useful. Women report stronger mental health gains, whereas men report greater improvements in physical health. These findings suggest that AI's workplace impact is uneven and shaped by demographic factors, career stage, and the nature of workers' roles.

The challenge of employee motivation in business management

This study investigates the role of employee motivation as a critical factor in effective business management and explores how financial and non-financial motivators shape engagement and performance. Based on a quantitative survey of 102 employees, the research analyzes differences in motivation levels across gender, age, and work experience, as well as the perceived effectiveness of various motivational tools. The findings indicate that financial incentives, particularly bonuses for achieving targets, are the most influential motivators, while non-financial factors such as flexible work schedules, additional leave, career development opportunities, and workplace atmosphere also play a crucial role in enhancing motivation. Significant variations in motivation were observed, with men, older employees, and those with longer tenure reporting higher levels. The study also reveals that work-life balance initiatives substantially increase motivation, highlighting the importance of combining financial and non-financial strategies to achieve optimal results. The results provide actionable insights for managers seeking to design effective motivation systems, showing that tailored, multifaceted approaches can improve employee satisfaction, retention, and organizational performance. Future research could explore cultural and sectoral differences and examine the evolving importance of motivational factors in remote and hybrid work environments.

Bias, Accuracy, and Trust: Gender-Diverse Perspectives on Large Language Models

Large language models (LLMs) are becoming increasingly ubiquitous in our daily lives, but numerous concerns about bias in LLMs exist. This study examines how gender-diverse populations perceive bias, accuracy, and trustworthiness in LLMs, specifically ChatGPT. Through 25 in-depth interviews with non-binary/transgender, male, and female participants, we investigate how gendered and neutral prompts influence model responses and how users evaluate these responses. Our findings reveal that gendered prompts elicit more identity-specific responses, with non-binary participants particularly susceptible to condescending and stereotypical portrayals. Perceived accuracy was consistent across gender groups, with errors most noted in technical topics and creative tasks. Trustworthiness varied by gender, with men showing higher trust, especially in performance, and non-binary participants demonstrating higher performance-based trust. Additionally, participants suggested improving the LLMs by diversifying training data, ensuring equal depth in gendered responses, and incorporating clarifying questions. This research contributes to the CSCW/HCI field by highlighting the need for gender-diverse perspectives in LLM development in particular and AI in general, to foster more inclusive and trustworthy systems.

Identifying social isolation themes in NVDRS text narratives using topic modeling and text-classification methods

Social isolation and loneliness, which have been increasing in recent years strongly contribute toward suicide rates. Although social isolation and loneliness are not currently recorded within the US National Violent Death Reporting System's (NVDRS) structured variables, natural language processing (NLP) techniques can be used to identify these constructs in law enforcement and coroner medical examiner narratives. Using topic modeling to generate lexicon development and supervised learning classifiers, we developed high-quality classifiers (average F1: .86, accuracy: .82). Evaluating over 300,000 suicides from 2002 to 2020, we identified 1,198 mentioning chronic social isolation. Decedents had higher odds of chronic social isolation classification if they were men (OR = 1.44; CI: 1.24, 1.69, p<.0001), gay (OR = 3.68; 1.97, 6.33, p<.0001), or were divorced (OR = 3.34; 2.68, 4.19, p<.0001). We found significant predictors for other social isolation topics of recent or impending divorce, child custody loss, eviction or recent move, and break-up. Our methods can improve surveillance and prevention of social isolation and loneliness in the United States.

Visual analysis of global research on the transient receptor potential ankyrin 1 channel: A literature review from 2002 to 2022

Background and aims: The transient receptor potential ankyrin 1 (TRPA1) channel has become a focus in pain research. However, there are no bibliometric studies that systematically analyze the existing research in this area. This study aimed to provide a systematic review of the existing literature on TRPA1 using a bibliometric analysis. Methods: Published literature in the field of TRPA1 was collected from the Web of Science Core Collection database. Quantitative and qualitative analyses of publications, countries, institutions, authors, journals, and other entries were conducted using Excel, VOSview, and Citespace software to provide insight into global research hotspots and trends in the TRPA1 field. Results: This study included 1189 scientific products published in 398 journals from 52 countries. The United States of America (n = 367) had the most publications, ahead of Japan (n = 212) and China (n = 199). The University of Florence (n = 55) was the most productive institution and Pierangelo Geppetti (n = 46) was the most productive author. PLoS One (n = 40) published the most articles on TRPA1. Pain, cold, inflammation, covalent modification, hyperalgesia, and oxidative stress were the most common keywords used in the studies. Conclusion: This study provides the first bibliometric analysis of TRPA1 publications. The physiological functions of TRPA1, TRPA1, and neuropathic pain, TRPA1 as a therapeutic target, and agonists of TRPA1 are trending in TRPA1 research. Neuropathic pain, apoptosis, and sensitization could be focus areas of future research. This study provides important insight in the field of TRPA1 research.

Examining Ibn Junaid's View about the Scope of hijab and Muslim Women's Shar'i Covering

Ibn Junaid Eskafi is one of the great Shiite scholars of the 4th century, whose books have not reached us, but some of his views have been quoted in jurisprudence books. Allameh Helii brought up a sentence of his about the clothing of men and women and he himself added an explanation to it, which has been misunderstood; To the extent that some non-experts have stated that Ibn Junayd considers the extent of obligatory covering for men and women to be equal in front of others! In this article, by an analytical method and by referring to the quoted sayings of Ibn Junayd, it becomes clear that such an understanding of Ibn Junayd's words was due to unfamiliarity with specialized jurisprudential texts, and Ibn Junayd did not believe in such a view not only regarding the women's veils in front of non-incest, but even in the "Prayer Veil"; Rather, the only difference between his opinion and others regarding women's veil is that he does not consider it obligatory to cover women's head during prayer, with the condition that non- incest does not see during prayer. Also, by carefully examining the above-mentioned quotes from him by Allameh Helii and others, it becomes clear that Ibn Junaid's sentence, which has been misunderstood, was only referring to the meaning of the word ``Awrat'', and if there is any ruling from it, it is the general ruling of veiling, which is also Both men and women are obliged to observe it in front of everyone (that is, even in front of their own sex and in front of incest); And his fatwa in this context is the same as the fatwa of the general Shia jurists.
