Immunodeficiency and hemolytic uremic syndrome: a case report

Background. Ataxia-telangiectasia (A-T) is a rare, autosomal recessive disorder characterized by cerebellar ataxia, oculocutaneous telangiectasia, and immunodeficiency, predisposing affected individuals to recurrent and severe infections. This case report presents a rare example of Shiga toxin-producing Escherichia coli (STEC)-associated hemolytic uremic syndrome (HUS) in a 12-year-old boy with a known diagnosis of A-T. To our knowledge, this is the first reported case of STEC-HUS in a patient with A-T. Case Presentation. The patient presented with vomiting and bloody diarhea Investigations revealed hemolytic anemia, thrombocytopenia, and acute kidney injury. The patient received intravenous immunoglobulin, albumin, and continuous renal replacement therapy and recovered. Conclusion. This case highlights the increased susceptibility of individuals with A-T to infections and the potential for life-threatening complications, such as HUS. The coexistence of A-T and STEC-HUS presentes unique challenges in diagnosis and management. Early recognition and targeted treatment led to a successful recovery and underscored the importance of close follow-up in immunodeficient patients.

Handwritten Collection of Irkutsk Archpriest Fortunat Petukhov: The Reconstruction of the Composition and Problems of Search

The purpose of the article is to present the handwritten collection of the Irkutsk archpriest Fortunat Ivanovich Petukhov (1789–1872), formed in the middle of the 19th century. The author has made an attempt 1) to reconstruct the composition of the manuscripts and documents of the collection, 2) to trace the ways of its formation, 3) to identify the features of the selection of manuscripts and to trace the degree of influence on the fate of the collection of the first presentation in the work by Moscow scholar N. P. Bocharov. Textual analysis of a number of works mentioned in the list has made it possible for other authors to attribute some of the texts presented by N. P. Bocharov as manuscripts of his collection, and not as the author’s works of Fr Fortunatus. We are talking about the well-known work of the Irkutsk local historian N. V. Semivsky, used for the autobiographical notes by F. I. Petukhov and as an integral part of other works by the archpriest, as well as about the manuscript of a much lesser-known work by another Irkutsk priest, Fr Nikifor Parnyakov. This gives reason to assume the possibility of a similar attribution of other works indicated by N. P. Bocharov with the name of F. I. Petukhov. Several manuscripts from the collection of F. I. Petukhov were published by the biographer as part of his book. The location of the collection of Archpriest Fortunatus Petukhov has not been found; the article suggests directions for searching for a possible repository – materials from the archival funds of Moscow monasteries, connected with the life and activities of people from Siberia – Zlatoust, Sretensky, etc., as well as searches for the personal archives of Fr Fortunatus’ descendants.

Писатели vs цензура: словенский опыт в контексте XIX в.

В рецензии представлена коллективная монография, опубликованная в научной серии «Studia literaria» Института словенской литературы и литературоведения Научно-исследовательского центра Словенской академии наук и искусств, в которой впервые комплексно, в мультидисциплинарном ключе представлено функционирование государственной цензуры как института официального контроля за социокультурной жизнью словенцев в период с 1789 по 1918 г. на территориях Габсбургской империи, а затем Австро-Венгрии. На протяжении всего рассматриваемого столетия словенские писатели, ученые, редакторы, издатели, критики, журналисты сталкивались с имперской цензурой как важнейшим фактором, регламентирующим их деятельность. Сам институт цензуры пережил в этот период ряд трансформаций: от секуляризации и обюрокрачивания во время правления Марии Терезии до репрессивной агрессивности цензурных практик при Франце Иосифе. Опираясь на широкий историко-культурный и литературный контекст, авторы – литературоведы, историки, искусствоведы, музейные работники, библиографы – исследуют влияние цензуры на процесс национального возрождения и творчество выдающихся словенских литераторов А. Т. Линхарта, Ф. Прешерна, И. Цанкара, на издательское и библиотечное дело, периодическую печать, национальный театр. Монография состоит из предисловия, обширного введения и двух расположенных в хронологическом порядке разделов, первый из которых охватывает период от Великой французской до Мартовской революции, второй – от Мартовской революции до Первой мировой войны. Каждый раздел включает десять авторских глав. Книга снабжена именным указателем, информацией об авторах и развернутым англоязычным резюме, включающим постатейные аннотации. Весьма информативен и полезен имеющийся в книге эксклюзивный архивный иллюстративный материал (копии документов, рукописей, писем, газетных страниц, цензурных заключений, судебных решений, географических карт, выписок из библиотечных каталогов и пр.), который не только служит доказательной базой, но и погружает читателя в атмосферу времени. Current apparent treatment‐resistant hypertension in patients undergoing peritoneal dialysis: A multi‐center cross‐sectional study

Abstract Apparent treatment‐resistant hypertension (aTRH) is the most commonly used term to report resistant hypertension (RH) and is considered as a common problem in dialysis population. However, few reports have focused on peritoneal dialysis (PD) hypertensive patients. The authors conducted a multi‐center cross‐sectional study involving 1789 PD patients from nine centers in Guangdong, China. The prevalence of aTRH was estimated by home blood pressure (BP) monitoring. Evaluating drug adherence through Eight‐item Morisky Medication Adherence Scale (MMAS‐8) and pill counting was performed to assess RH in one PD center. Related factors of aTRH were analyzed using logistic regression analysis. The prevalence of aTRH in PD patients was estimated at 42.2% (755 out of 1789 hypertensive patients) based on home BP. Of those, 91.4% patients were classified as uncontrolled RH, 2.0% as controlled RH, and 6.6% as refractory hypertension. The prevalence of RH was 40.6% and 41.9% among those with medium/high adherence based on the MMAS‐8 scores and the pill counting rate, respectively. PD patients who were younger, with higher body mass index, with lower serum albumin and poorer dialysis adequacy were significantly associated with higher aTRH incident. In conclusion, the present study demonstrates a high prevalence of aTRH in PD population, which occurs in about two in five treated hypertensive patients. Nutritional status and dialysis adequacy might tightly associate with aTRH.

Negozi in serie nell’Italia tra le due guerre. Lagomarsino e l’architettura pubblicitaria

La ditta milanese Lagomarsino è stata una delle più importanti produttrici di macchine calcolatrici, addizionatrici e contabili. Nell’ambito di una vera e propria strategia di comunicazione per la vendita, all’inizio degli anni Quaranta l’azienda affidò la progettazione dei suoi negozi in tutta Italia ad Angelo Bianchetti e a Cesare Pea. Grazie all’esperienza acquisita nel campo dell’architettura pubblicitaria, i due architetti svolsero il compito adottando un riconoscibile codi-ce comunicativo e utilizzando elementi plastici e grafici, molto più simili a installazioni temporanee che a tradizionali arredamenti commerciali. Basato su documenti inediti e fonti a stampa poco note, il saggio prende in esame il caso Lagomarsino, confrontandone le strategie comunicative con quelle adottate nei negozi del suo più celebre competitor, la Olivetti, alla luce dei più generali rapporti tra architettura, design, arte e pubblicità, caratterizzanti il rinnovamento dell’estetica dei luoghi per la vendita in Italia tra le due guerre. The Milanese firm Lagomarsino was one of the most important manufacturers of calculating, adding and accounting machines. In the early 1940s, as part of a marketing communication strategy, the company entrusted the design of its stores in Italy to Angelo Bianchetti and Cesare Pea, who had gained experience with advertising architecture. The two architects adopted a recognizable style, using plastic and graphic elements that were much more frequently used in temporary installa-tions than in traditional store fittings. Using unpublished documents and little-known print sources, this essay discusses the Lagomarsino case, comparing its communication strategies to those employed in the stores of its renowned competitor, Olivetti. The analysis will be carried out in light of the relationship between architecture, design, art and advertisement, which characterize the new store aesthetics in Italy between the two wars.

Prospectiva de la producción de coco en Yucatán, México

Objetivo: realizar una prospectiva de la producción de coco en Yucatán. Metodología: se llevó a cabo una investigación documental y se efectuaron encuestas y entrevistas a productores y a otros actores de las regiones productoras de coco de Yucatán. Resultados: la recuperación de la producción del fruto en el estado ha sido muy lenta, porque después de la devastación que causó la enfermedad del amarillamiento letal del cocotero, los actores quedaron desarticulados y desorganizados. Además, hubo escaso aprovechamiento integral de la fruta, falta de créditos del gobierno o apoyos económicos privados. No obstante, en el estado hay desarrollos científicos que pueden contribuir a la creación de plantaciones productivas. Limitaciones: el trabajo de campo se realizó en al menos un municipio por cada región productora. Se seleccionaron aquellos que tenían mayor producción. Valor: se analizó la actual producción y se hizo una prospectiva de los escenarios posibles. Conclusiones: es importante la creación de vínculos entre productores, académicos y gobierno, dar capacitación sobre el aprovechamiento integral del fruto e impulsar políticas públicas para minimizar las debilidades de la cadena de producción y maximizar las oportunidades para los productores.

Original y copia de un Goya. El Retrato de Carlos IV del antiguo Hospital Real de Santiago de Compostela

Este estudio ofrece un avance importante en la identificación y documentación del proceso de copia pictórica en la serie de los retratos oficiales de Carlos IV, pintados por –o bajo supervisión de– Francisco de Goya entre 1789 y 1790. Los autores analizan uno de estos retratos, fechado en 1790, conservado en el ayuntamiento de Santiago de Compostela, y llevan a cabo el estudio comparativo –especialmente el dibujo subyacente– de este con otro retrato de Carlos IV, hasta ahora prácticamente desconocido, que es en verdad el primero de la serie. [gl] Este estudo ofrece un avance importante na identificación e documentación do proceso de copia pictórica na serie de retratos oficiais de Carlos IV, pintados por –ou baixo a supervisión de– Francisco de Goya entre 1789 e 1790. Os autores analizan un destes retratos, datado en 1790, conservado no concello de Santiago de Compostela, e levan a cabo o estudo comparativo –especialmente o debuxo subxacente– deste outro retrato de Carlos IV, ata agora practicamente descoñecido, que en realidade é o primeiro da serie.

ACȚIUNEA ȘI INACȚIUNEA CA FORME ALE FAPTEI PREJUDICIABILE PREVĂZUTE LA art. 146 DIN CODUL PENAL

<p>Oricare acțiune de lipsire ilegală de viață a unei alte persoane, posibilă în condițiile aflării făptuitorului în stare de afect, poate exprima conținutul faptei prejudiciabile prevăzute la art.146 din Codul penal al Republicii Moldova. Impor­­tant este ca o astfel de acțiune să demonstreze spontaneitate și să nu presupună crearea prealabilă de condiții pentru lipsirea ilegală de viață a victimei. Infracțiunea prevăzută la art.146 CP RM poate fi comisă pe calea inacțiunii. În cazul omorului în stare de afect comis prin inacțiune, obligația făptuitorului de a menține viața victimei este stabiliă de un act normativ extrapenal, care fie constituie lege, fie dezvoltă o lege, fără a crea reglementări de sine stătătoare. Criteriul de delimitare a infracțiunii prevăzute la art.146 CP RM de infracțiunile prevăzute la art.162 sau 163 din CP RM este urmă­torul: în situaţia omorului săvârșit în stare de afect, făptuitorul doreşte sau admite moartea victimei; în cazul neacor­dării de ajutor unui bolnav sau al lăsării în primejdie, făptuitorul dorește producerea pericolului morții. În cazul infracțiunilor prevăzute la art.162 sau 163 CP RM, față de moartea victimei făptuitorul poate manifesta exclusiv imprudență. Dispozi­țiile extrapenale pot doar contribui la interpretarea art.146 CP RM, însă nu pot stabili temeiul juridic al răspunderii penale pentru neexecutarea de către făptuitor a obligației de a menține victima în viață.</p><p> </p><p><strong>ACTION AND INACTION AS FORMS OF THE PREJUDICIAL </strong></p><p><strong>ACT SET FORTH IN art. 146 OF THE PENAL CODE</strong></p><p>Any action of unlawful deprivation of life of another person, possible under the condition of the perpetrator being in the heat of passion, can express the content of the prejudicial act set forth in art.146 of the Penal Code of the Republic of Moldova. It is important that such an action demonstrates spontaneity and does not presuppose the creation of conditions for the illegal deprivation of life of the victim. The crime set forth in art.146 PC RM can be committed by inaction. In the case of murder in the heat of passion committed by inaction, the perpetrator's obligation to maintain the life of the victim is established by an extra-penal normative act, which either constitutes a law or develops a law, without creating regulations on its own. The criterion of delimitation between the crime set forth in art.146 PC RM and the crimes set forth in art.162 or 163 PC RM is the following: in the case of murder in the heat of passion, the perpetrator wishes or admits the death of the victim; in the case of withholding help from a sick person or abandonment in danger, the perpetrator wishes to produce the danger of death. In the case of the crimes set forth in art.162 or 163 PC RM, regarding the death of the victim, the perpetrator can only express imprudence. The extra-penal provisions can only contribute to the interpretation of art.146 PC RM, but cannot establish the legal basis of the criminal liability for the non-execution by the perpetrator of the obligation to keep the victim alive.</p>

Escarabajos (coleópteros) de Perú: un reconocimiento de las familias. Attelabidae Billberg, 1820

Se describe una nueva especie, Omolabus (Sternolaboides) sokolovi Legalov n. sp. de Satipo, centro de Perú. Se registran nuevos datos para Hybolabus ater (Olivier, 1789), Omolabus (Perulabus) peruanus Legalov, 2004, O. (Pseudomolabus) westerduijni Legalov, 2008 y O. (Sternolaboides) ecuadorensis Legalov, 2007. Basada en la literatura y exámenes de muestras, se elabora una lista de las 18 especies peruanas de Attelabidae, pertenecientes a ocho géneros de dos tribus, tambien se presentan sus distribuciones en Perú. Immature stages of three species of Inca LePeletier Serville, 1828 (Coleoptera: Scarabaeidae: Cetoniinae) and morphology of phytophagous scarab beetle pupa.

Immature stages of I. pulverulenta (Olivier, 1789) (Coleoptera: Scarabaeidae: Cetoniinae: Incini) are described for the first time, and those of I. bonpalandi (Gyllenhal, 1827) and I. clathrata sommeri Westwood, 1845 are redescribed. A key to the third instar of Incini is presented, notes on Inca larval characters in the phylogenetic context of Cetoniinae and on scarab pupal morphology are also given.

Effect of nocturnal hemodialysis on sleep parameters in patients with end-stage renal disease: a systematic review and meta-analysis.

INTRODUCTION:Recently, a small but growing literature has depicted the beneficial effects of nocturnal hemodialysis (NHD) over conventional hemodialysis (CHD) in the fields of sleep disorders such as sleep apnea. The impact of various dialysis models on sleep disorders, however, has not been determined. The objective of our meta-analysis is to examine the potential effects of NHD, compared with CHD, on sleep disorders in HD patients. METHODS:Several electronic databases including PubMed, EMBASE, Cochrane Central Register of Controlled Trials, ClinicalTrials.gov and CNKI were searched, using the search terms "nocturnal" (or "nightly") and "dialysis" (or "hemodialysis" or "renal dialysis") from the earliest available date of indexing to March 2018. Two authors independently extracted data, evaluated the study quality, and conducted random-effects meta-analyses using STATA 12.0. RESULTS:Of 1789 potentially relevant citations, 9 fulfilled eligibility criteria, consisting of 6 single-arm studies (comparing pre- and post-intervention outcomes), 1 observational study, and 2 randomized controlled trials (a total of 286 participants). Regarding objective sleep assessments, conversion from CHD to NHD resulted in a significant reduction in the AHI (Mean difference was -14.90; 95% CI, -20.12 to -9.68), a significant increase of SaO2 (Mean difference was 1.38%; 95% CI, 0.35% to 2.42%), and a significant decrease of TST (Mean difference was -0.31; 95% CI, -0.47 to -0.15). The trends were even stronger in the HD patients with sleep disorders. However, regarding subjective sleep assessments, improved sleep quality was found in the prospective pre-post intervention studies and cohort studies, while no significant improvements were found in the randomized controlled trials. CONCLUSION:Although a significant improvement of sleep apnea was observed by switching from CHD to NHD, it may not yield a net benefit in overall subjective sleep quality.

El paper de les ocupacions de terra en el procés d’estructuració del MST (1984-1995)

En aquest article es discuteixen les transformacions en el Moviment dels Treballadors Rurals Sense Terra (MST) a partir de la definició del caràcter estratègic i multiforme de l’ocupació de terra com a forma de lluita, instrument per a l’expansió del Moviment, espai de formació i element de distinció de les altres organitzacions polítiques. A través de la literatura especialitzada i d’un conjunt d’entrevistes fetes amb els actors del procés estudiat, s’aprofundeix en els lideratges que es van formular i que van portar a terme les ocupacions de terres durant els anys de 1980, especialment en el Nord-est brasiler. Són analitzades les disputes, les deliberacions, la relació amb l’Estat i les conquestes materials i simbòliques derivades de les ocupacions de terra en territori nacional.

Updated catalogue and taxonomic notes on the Old-World scorpion genus Buthus Leach, 1815 (Scorpiones, Buthidae)

Abstract Since the publication of the ground-breaking “Catalogue of the scorpions of the world (1758–1998)” (Fet et al. 2000) the number of species in the scorpion genus Buthus Leach, 1815 has increased 10-fold, and this genus is now the fourth largest within the Buthidae, with 52 valid named species. Here we revise and update the available information regarding Buthus. A new combination is proposed: Buthus halius (C. L. Koch, 1839), comb. n. from Portugal and Spain. B. halius is removed from junior synonymy with Buthus occitanus (Amoreux, 1789), and proposed as a senior synonym of B. ibericus Lourenço & Vachon, 2004, syn. n. Moreover, following I.C.Z.N. article 23.9.2 we propose to maintain as valid B. ibericus (nomen protectum) and to consider the disued B. halius as a nomen oblitum. Buthus europaeus tridentatus Franganillo, 1918 is proposed as a junior synonym of B. occitanus (Amoreux, 1789), syn. n. Buthus sabulicola Touloun, 2012 is proposed as a junior synonym of Buthus bonito Lourenço & Geniez, 2005, syn. n. Buthus occitanus tunetanus neeli Gysin, 1969 is proposed as an informal senior synonym of Buthus tassili Lourenço, 2002, informal syn. n. Two taxa are rised to species rank, Buthus nigrovesiculosus Hirst, 1925, stat. n. and Buthus parroti Vachon, 1949, stat. n.. We further confirm the restricted distribution of B. occitanus that is confined to southeastern France and northwestern Iberian Peninsula and does not occur in North Africa. Additionally, Androctonus barbouri (Werner, 1932), comb. n. from the Agadir region of Morocco, is hereby transferred to the genus Androctonus. We summarize and provide a critical appraisal of the diagnostic characters currently in use for the genus. The catalogue section considers the names for species, subspecies and varieties that have been used for Buthus scorpions. Information about types, including collection numbers and localities are included when available. Finally, an annotated listing of synonymies and an updated bibliography are given.

The cultural evolution of national constitutions

We explore how ideas from infectious disease and genetics can be used to uncover patterns of cultural inheritance and innovation in a corpus of 591 national constitutions spanning 1789–2008. Legal “ideas” are encoded as “topics”—words statistically linked in documents—derived from topic modeling the corpus of constitutions. Using these topics we derive a diffusion network for borrowing from ancestral constitutions back to the US Constitution of 1789 and reveal that constitutions are complex cultural recombinants. We find systematic variation in patterns of borrowing from ancestral texts and “biological”‐like behavior in patterns of inheritance, with the distribution of “offspring” arising through a bounded preferential‐attachment process. This process leads to a small number of highly innovative (influential) constitutions some of which have yet to have been identified as so in the current literature. Our findings thus shed new light on the critical nodes of the constitution‐making network. The constitutional network structure reflects periods of intense constitution creation, and systematic patterns of variation in constitutional lifespan and temporal influence.
