Efficacy of Mamsyadi Ghana capsule and Saraswatarista in the management of Insomnia Disorder - A Randomized Controlled Clinical Trial

Background: One of the prevalent sleep disorders is insomnia disorder. Current pharmacotherapy have limitations like dependence, tolerance and withdrawal effects. Ayurveda drugs remains to be explored. Objectives: Can Ayurveda intervention (Mamsyadi Ghana Capsule and Saraswatharista) play a role in Insomnia disorder. Material and Methods: 50 patients meeting the diagnostic criteria of Insomnia Disorder (DSM 5), age group 20–70 years, either sex participated in the study. They were randomly divided and Ayurveda group received Mamsyadi Ghana Capsule 500 mg, Saraswatharista 10 ml, with equal quantity of water after food, both thrice a day for 30 days. Zolpidem group received tablet Zolpidem 10 mg once a day at bed time. Evaluations were done through Insomnia severity Index (ISI), Pittsburgh Sleep Quality Index (PSQI), Epworth Sleepiness scale (ESS), Sleep diary parameters, Depression Anxiety and Stress Scale (DASS), Quality of Life – SF 36 Questionnaire and UKU side effect rating scale. Clinical assessments were at baseline, 15th and 30th day. Results: The study showed that Ayurveda intervention produced significant improvement and was comparable to Zolpidem. Both interventions were comparable in ISI, PSQI, ESS, DASS scale, sleep diary parameters and most of the components of SF-36. Role of limitations due to emotional problems component showed better improvement in Ayurveda group (p = 0.01). Within group analysis showed improvement in both the groups. Five patients had adverse effects with Zolpidem and nil with ayurveda group. Conclusions: The study showed that effect of Ayurveda interventions (Mamsyadi Ghana Capsule and Saraswatharista) is comparable to zolpidem in Insomnia Disorder. Ayurveda intervention had better improvement on quality of life and had good safety profile. Ctri registration number: CTRI/2021/12/038809.

Effects of Android phone vs. iPhone use on BlackBerry thumb symptoms among university students in Bangladesh

Abstract Background In Bangladesh, the most prevalent musculoskeletal condition among office employees is considered as BlackBerry thumb (BBT). Alike official perspectives, our educational system was significantly regulating with the digital interfaces at COVID-19 lockdown, where a greater reliance on Android phones were experienced among the adults. Numerous studies have been conducted in studying the incidences of BBT in young individuals as a result of hazards of Android phone usage (HAPU) in Bangladesh. Objective This research sought to determine the relationship between BBT symptoms and the risks associated with the using Android phones among Bangladeshi university students. Methods A nationwide cross-sectional study was undertaken on a group of university students between the ages of 18 and 25 to determine if BBT symptoms were present based on the Finkelstein test and HAPU, which were also assessed using a well-designed questionnaire. We calculated the crude and adjusted prevalence ratios (aPR) and used a generalized linear model from the Poisson family, using their respective 95% confidence intervals (CI). Results There were 2455 individuals in this research, with a median age of 20 and an interquartile range (IQR) of 19 to 23. Of them, 1185 males (48.27%) and 1270 women (51.75%) made up the study’s participant population. Physical exams showed that 1300 individuals had positive Finkelstein test results (52.95%), whereas 1040 people had occasional risks from using an Android phone and 115 participants had occasional risks from using an iPhone. In our generalized linear model, we observed that participants with occasional and frequent HAPU had higher rates of BBT symptoms than responders without HAPU (aPR = 1.73, 95% CI: 1.47–2.05, and aPR = 1.61, 95% CI: 1.29–2.00), respectively. Conclusion The current study found that Bangladeshi university students experiencing BlackBerry thumb symptoms were more likely to have risks associated with using Android phones.

TELEODONTOLOGIA NA EDUCAÇÃO DE PROFISSIONAIS E NO CUIDADO NO CONTEXTO DA ATENÇÃO BÁSICA À SAÚDE NO BRASIL

Objetivo: Analisar o uso da teleodontologia na Atenção Básica à Saúde (ABS), na perspectiva da educação de profissionais e do cuidado em saúde no Brasil. Metodologia: Foi realizada uma revisão integrativa da literatura com consulta às bases de dados da Biblioteca Virtual em Saúde (BVS) e National Library of Medicine (PubMed). Descritores controlados junto ao Dicionário de Descritores em Ciências da Saúde foram combinados, nos idiomas português e inglês (teleodontologia/teledentistry, Brasil/Brazil, Atenção Primária à Saúde/Primary Health Care; educação/education; assistência odontológica/dental care). Os critérios de inclusão contemplaram artigos científicos (ensaios, editoriais, comentários, revisões de literatura, relatos de experiência/caso, pesquisas empíricas) relacionados ao tema do estudo, publicados de 2007 a março de 2022. Foram excluídas publicações duplicadas, cartas e trabalhos acadêmicos. Resultados: Foram analisados 17 artigos, publicados de 2008 a 2021. Os resultados mostraram a teleodontologia utilizada na ABS em ações de tele-educação (cursos a distância, videoconferências), telediagnóstico (diagnóstico de lesões orais), teleconsultorias (apoiando profissionais da saúde em dúvidas e encaminhamentos), telemonitoramento de pacientes e teleconsultas durante a pandemia de COVID-19. Sua utilização foi associada ao fortalecimento da ABS, aumento da resolubilidade em saúde, da coordenação de encaminhamentos e do acesso aos serviços de saúde a pacientes moradores de áreas rurais. Falta de treinamento, resistência dos profissionais em relação ao uso de tecnologias da informação e comunicação, acesso limitado à internet, fragilidades de infraestrutura e questões relacionadas à segurança dos pacientes, foram barreiras identificadas relacionadas à utilização da teleodontologia. Conclusão: A teleodontologia foi uma ferramenta utilizada e valorizada no contexto do cuidado em saúde bucal na ABS para profissionais e pacientes, ampliando o acesso e sendo resolutiva ao reduzir os encaminhamentos às especialidades odontológicas. Tem potencial de crescimento e expansão no Sistema Único de Saúde, desde que as barreiras sejam consideradas.

Development of a microfluidic system with a bearing micropump for applying fluid shear force in an intervertebral disc degeneration model involving mechanical stimulation

Background The intervertebral disc (IVD) functions as a shock absorber with viscoelastic tissues, including the gelatinous nucleus pulposus and the collagenous annulus fibrosus (AF). External mechanical loading influences IVD homeostasis but can cause damage and inflammatory reactions, contributing to disc degeneration. There is a lack of in vitro platforms for modeling IVD disease with mechanical stimulation. Methods This study aimed to create a mechanical stimulation-based model by subjecting AF cells to shear stress using a micropump system. A micropump platform was used to measure pulsation, flow rate, and shear stress. AF cells were exposed to fluid shear stress of 0.5 and 1 dyne/cm², and morphological changes, cytotoxicity, and cell viability were analyzed through a live/dead assay. Perimeter, area, diameter, and elongation were assessed using live cell imaging and imaging analysis software. Results The micropump platform exhibited optimal rotor characteristics for uniform pulsation and shear stress. Fluid shear stress at 0.5 dyne/cm² showed no significant difference from the control group, while 1 dyne/cm² reduced cell adhesion and survival. Comparing the 0.5 dyne/cm² shear stress group and interleukin-1β group to the control, significant decreases in perimeter, area, and diameter were observed. Conclusion The study successfully developed a micro-peristaltic pump platform for applying fluid shear stress to cells, identifying an optimal rotor for uniform stress application. The platform effectively modeled IVD disease, revealing reduced cell adhesion and survival under specific shear stress conditions. This platform has potential promise for discovering biomarkers and exploring cellular responsiveness to external stimuli.

Impact of yoga intervention in lower limb amputees following trauma in relation to behavior and quality of life: A randomized controlled trial

Background: Yoga is an emerging intervention causing improvement in physical, mental, and spiritual well-being. Its role in improving outcomes in patients with amputation was investigated. Methodology: Patients with traumatic lower limb amputation (n = 50) were enrolled and randomized to the Yoga group (n = 26) against control (n = 24). Sociodemographic details, quality of life (QOL) (World Health Organization QOL-BREF), Depression anxiety stress scale (DASS), Rosenberg self-esteem scale, and amputee body image score were applied at baseline, 6,–18 weeks of amputation. Results: Sociodemographic and clinical variables were comparable between groups. At 18 weeks, the Yoga group had better QOL (P = 0.005) than the control group. Symptoms of depression (0.02) and anxiety (<0.001) reduced, and self-image (P = 0.015) improved significantly at 6 weeks, while stress (P = 0.003) reduced at 18 weeks in the yoga group. Despite comparable body image scores, the prosthesis usage (hours/day) was more (P = 0.005) in the Yoga (6.9 ± 3.2) group against the control (12 ± 2.7). Conclusion: Yoga improves QOL and self-esteem and reduces depression, anxiety, and stress symptoms in patients with traumatic amputation.

FORMAÇÃO EM DIABETES PARA PROFISSIONAIS DA EDUCAÇÃO DE BLUMENAU/SC

O Diabetes Mellitus tipo 1 é a forma mais frequente de diabetes em crianças e adolescentes. Seu tratamento inclui múltiplas intervenções que exigem apoio ao cuidado. Por passarem uma parte significativa do dia na escola, as ações intersetoriais, neste caso, saúde e educação, são imprescindíveis para o apoio desta condição de saúde durante o horário escolar. No entanto, profissionais da educação tem pouco ou nenhum preparo para apoiar o gerenciamento do diabetes. Nesse contexto, a integração ensino-serviço e a Educação Permanente em Saúde são ferramentas de transformação de práticas profissionais. Nesta perspectiva, o projeto de extensão Doce Alegria e o Programa de Pós-Graduação de Saúde Coletiva da Universidade Regional de Blumenau propõem uma formação em diabetes para profissionais da rede de ensino do município de Blumenau-SC. Os conteúdos foram organizados em três módulos: a) conhecendo o diabetes; b) auxiliando os estudantes com diabetes e c) adquirindo habilidades técnicas. A carga horária será de dez horas e a metodologia a ser utilizada será a problematização com uso do arco de Mangarez. A avaliação abrangerá os conhecimentos em diabetes (pré e pós-formação), a formação em si e o seu impacto no cotidiano escolar. A articulação com os profissionais da escola terá apoio da Secretaria Municipal de Educação de Blumenau. Estima-se que se inicie um processo de transformação da realidade a partir do desenvolvimento de competências para o apoio ao cuidado em diabetes contribuindo com a política nacional de atenção integral à saúde da criança e ao programa saúde na escola.

Perceptions of using herbal medicines for managing menopausal symptoms: a web-based survey of Korean medicine doctors

Background: Most women experiencing menopause have sought out traditional treatment options to relieve their symptoms. The aim of this study, which used a web-based survey, was to investigate perceptions of Korean medicine doctors (KMDs) on managing menopausal symptoms with herbal medicines. Methods: We developed and then sent a questionnaire via email to a total 497 KMDs in obstetrics and gynecology affiliated with the Society of Korean Medicine. The questionnaire consisted of three categories and a total of 26 items focusing on major menopausal symptoms, prescriptions, pattern identification, and background characteristics. Results: Out of the 497 KMDs, we received responses from 48 doctors, a response rate of 9.7%. The most common symptoms of menopausal disorder perceived by responded KMDs were hot flashes (22.9%), sleep disorders (18.2%), systemic fatigue (12.5%), and sweating (12.0%). The herbal medicines reported to be the most effective when treating patients with menopausal disorders were Jiawei shaoyao-san (12.5%), Jiawei guipi-decoction (11.8%). The responding KMDs recommended one to two months of treatment for female menopausal disorder, using two to three doses of herbal medicine per day in combination with traditional Korean medicine (KM) therapies such as acupuncture, moxibustion, cupping, and pharmacopuncture. Conclusion: The results of this survey may provide information about doctor’s perceptions on using herbal medicines to manage patient’s menopausal symptoms. Future studies should be done with a larger sample size to overcome the limitation of this study and to examine how KM is used for treating menopausal symptoms in clinical practice. Este texto traz reflexões de experiência discente sobre os temas ‘ensino e saúde’ a partir de uma disciplina de um curso de mestrado que aproximou filmes e o ensino na saúde. O uso deste recurso audiovisual convida a todos para a experiência de rever o processo de ensino e aprendizagem, deslocando o pensamento ao observar outras relações que se dão no encontro entre mestres e estudantes, por vezes invertendo o lugar de quem ensina e quem aprende. O objetivo deste texto é explorar uma visão sobre filmes que por cenas, frases ou por seu conjunto, traçou relações com o aprender-ensinar. De poetas e músicos a rebeldes e bruxos, as correlações se descortinam entre ensinar e aprender, e principalmente de viver. Neste texto, exemplos da sétima arte instigam a (re)pensar o quanto podem ser fluidos, polimorfos e inovadores os processos de aprendizagem e o quanto estes processos são pertinentes ao fazer em saúde: inovador, inusitado, singular a cada ato de trabalho, de ensino e de aprendizagem.

Bodily differences between Cold- and Heat-prescription groups in Sasang medicine

Background: In Sasang medicine, patients are treated with herbal prescriptions based on Cold-Heat patterns induced by conditions of physiological equilibrium among internal organs, which also induce differences in body circumferences. The objective of the current study was to elucidate the relationship between Cold- and Heat-prescription types and body circumferences.
Methods: Data from suitable subjects (115 males and 222 females) were collected, and the body circumferences were compared according to their Cold- and Heat-prescription grouping. For male subjects with significant body circumference differences between the Cold- and Heat-prescription groups, the body circumference ratios were analyzed by comparing the differences between these groups using ranked analysis of covariance (ANCOVA).
Results: In men, consistent differences were observed in the body circumference measures and ratios between the Cold- and Heat-prescription groups. The Heat-prescription groups showed greater abdominal circumferences, notably demonstrated by a higher rib-to-pelvic circumference ratio in the Tae-Eum (TE) type (p = 0.041) and a higher chest-to-hip circumference ratio in the So-Yang (SY) type (p = 0.087).
Conclusion: In the SY-type men, the chest circumference was significantly greater in the Heat-prescription group compared to the Cold-prescription group. In the TE-type men, the rib-to-pelvic circumference ratio was significantly higher in the Heat-prescription group than in the Cold-prescription group.

Ayurvedic management of spondyloepiphyseal dysplasia tarda, a rare hereditary disorder

Spondyloepiphyseal dysplasia tarda (SEDT) is a rare genetic disease in which patient suffers from short stature, short trunk and neck with disproportionately long arms, coxa vara, skeletal features such as barrel shaped chest, kyphosis, scoliosis and early arthropathy. Only limited medical and surgical management is available in modern medicine. A 15 years old male suffering from SEDT and diagnosed as Vata vyadhi was treated with Panchakarma therapy and selected Ayurvedic oral medicines. Ayurvedic treatment was directed to ameliorate the orthopaedic clinical conditions in this case. Panchakarma procedures such as Shalishastika pinda svedana for a month and Mustadi yapana basti for 16 days were given along with oral Ayurvedic medicines. Same Panchakarma procedures were repeated after an interval of 2 months. A combination of Ayurvedic oral medicines such as Trayodashanga guggulu-500 mg twice a day, Dashmool kvatha (decoction of roots of 10 herbs) 40 ml twice a day, Eranda paka 10 g twice a day, Shiva gutika-500 mg twice a day and Dashmoolarista-20 ml (with equal water) twice a day were prescribed. Eight scales based Medical outcome study (MOS) – 36 item short form – health surveys was assessed for outcome which shows good improvement. Kyphosis, scoliosis and pain were moderately reduced. Clinical experience of this case indicates that Ayurvedic herbs along with Panchakarma can play a major role in the management of hereditary disorder SEDT.

Electrophysiological properties and calcium handling of embryonic stem cell-derived cardiomyocytes

Embryonic stem cell-derived cardiomyocytes (ESC-CMs) hold great interest in many fields of research including clinical applications such as stem cell and gene therapy for cardiac repair or regeneration. ESC-CMs are also used as a platform tool for pharmacological tests or for investigations of cardiac remodeling. ESC-CMs have many different aspects of morphology, electrophysiology, calcium handling, and bioenergetics compared with adult cardiomyocytes. They are immature in morphology, similar to sinus nodal-like in the electrophysiology, higher contribution of trans-sarcolemmal Ca2+ influx to Ca2+ handling, and higher dependence on anaerobic glycolysis. Here, I review a detailed electrophysiology and Ca2+ handling features of ESC-CMs during differentiation into adult cardiomyocytes to gain insights into how all the developmental changes are related to each other to display cardinal features of developing cardiomyocytes.

Multi-centric clinical study of the effect of intervention time on efficacy of gastroparesis external application prescription treatment of gastrointestinal tumor postsurgical gastroparesis

Objective: This intervention has been shown to be clinically efficacious and safe. The aim of this study was to determine the effect of different intervention times on the efficacy of gastroparesis external application prescription versus placebo for the treatment of gastrointestinal tumor postoperative, postsurgical gastroparesis.
Methods: A multicenter, randomized, double-blind, placebo controlled phase III clinical trial was designed to demonstrate superiority and conducted at four grade 3A hospitals in Beijing. Patients diagnosed with gastrointestinal tumor postsurgical gastroparesis and local cold syndrome in the abdomen were enrolled and received conventional treatment (nutritional support, gastrointestinal decompression, and prokinetic medication). Treatment and control groups respectively received gastroparesis external application prescription or placebo acupoint application over a treatment course of 14 days or until the primary efficacy endpoint (clinical efficiency) was achieved.
Results: A total of 120 patients were enrolled in the treatment and control groups (n = 60 per group), and 15 patients dropped out of the study because of skin allergies (n = 7) or poor efficacy (n = 8). The efficacy among patients in Class B of Group A(treatment group) was marginally better than that of Group B (control group) (64.28% vs. 55.56%) although the difference between the two groups was not significant. However, the efficacy among patients in Class C of Group A was significantly better than that of Group B (79.49% vs. 43.33%). For Group A, the time for patients in the three classes to achieve the efficacy endpoint increased significantly with disease progression (3.00, 6.78, and 8.29 days for Class A, B, and C, respectively).
Conclusions: Gastroparesis external application prescription can effectively treat gastrointestinal tumor postsurgical gastroparesis and may be more efficacious in progressive disease compared placebo. Patients with gastrointestinal tumor postsurgical gastroparesis should therefore undergo intervention at an earlier stage.

Effectiveness of yogic breathing intervention on quality of life of opioid dependent users

Introduction: The quality of life (QOL) of substance users is known to be impaired. Sudarshan Kriya Yoga (SKY), a yogic breathing program has potential to improve QOL and needs evaluation in an Indian setting.
Aims: Study aimed to assess changes in QOL in treatment seeking male opioid dependent users following practice of SKY program.
Settings and Design: Users were randomized into study (n = 55) and control group (n = 29). Study group besides standard treatment (long term pharmacotherapy with buprenorphine in flexible dosing schedule) underwent a 3 days, 12 h SKY program while control group received standard treatment alone.
Materials and Methods: World Health Organization QOL-brief scale was used to measure QOL and urine tested to assess recent drug use. Assessments were made at baseline and at 3 and 6 months.
Statistical Analysis: Data were analyzed using generalized estimation equation to assess within group change with time and the overall difference between groups for changes at assessment points.
Results : Overtime within study group, all four QOL domain scores were significantly higher at 6 months. Between group comparison showed significant increase in physical (P < 0.05); psychological (P < 0.001) and environment domains (P < 0.001) for study group while control group showed significant changes in social relationship domain only. Urine screening results were negative for study group indicating no drug use at 6 months.
Conclusion: SKY as a complementary therapy was found beneficial in improving QOL for group practicing it and is recommended for use as low cost and low-risk adjunct in substance treatment settings in India.

Single-dose Toxicity of ShinYangHur Herbal Acupuncture

Objectives: This study was carried out to analyze the single-dose toxicity of ShinYangHur (SYH) herbal acupuncture injected into the muscles of Sprague-Dawley (SD) rats. Methods: The SYH herbal acupuncture was made in a clean room at the Korean Pharmacopuncture Institute (KPI, Korea-Good Manufacturing Practice, K-GMP). After the mixing process with sterile distilled water, the pH was controlled to between 7.0 and 7.5. Then, NaCl was added to make a 0.9% isotonic solution by using sterilized equipment. All experiments were conducted at Biotoxtech, an institution authorized to perform non clinical studies under the regulations of Good Laboratory Practice (GLP). SD rats were chosen for the pilot study. Doses of SYH herbal acupuncture, 0.25, 0.5, and 1.0 mL, were administered to the experimental groups, and a dose of normal saline solution, 1.0 mL, was administered to the control group. This study was conducted under the approval of the Institutional Animal Ethics Committee. Results: No deaths or abnormalities occurred in any of the four groups. No significant changes in weight, hematological parameters or clinical chemistry between the control group and the experimental groups were observed. To check for abnormalities in organs and tissues, we used microscopy was used to examine representative histological sections of each specified organ; the results showed no significant differences in any of the organs or tissues. Conclusion: The above outcomes suggest that treatment with SYH herbal acupuncture is relatively safe. Further studies on this subject are needed to yield more concrete evidence.
