Beyond “Ch'jurnaaq”: Recognizing Maya Indigenous People's perceptions of psychosis and recovery in Santiago Atitlán, Guatemala

Michelle Marie Pieters, Diego Sapalú, Alex Cohen et al.

Introduction: Psychotic conditions are major causes of disability in Latin America, where mental healthcare is mostly limited and inadequate. Indigenous People throughout this region experience additional barriers to mental health care, compounded by structural inequities and diverse health practices. This study in Guatemala described the Maya Indigenous People's perceptions about psychosis, its symptoms, causes, responses to, and recovery. Results will inform the design of a community-led, culturally grounded program for people living with psychosis (PLWP). Methods: We conducted free listing and semi-structured interviews in Spanish or Maya Tz'utujil language. Participants were PLWP, family caregivers, community leaders, and mental health, traditional, and primary care providers. We followed Kleinman's explanatory model and elicited locally meaningful recovery outcomes. Our team performed a matrix-based thematic analysis. Results: We interviewed 30 participants. Key findings included (1) varying terms and understandings of psychosis symptoms and causes across participant subgroups; (2) extremely limited care, pluralistic treatment practices, and tensions between biomedical and traditional systems; (3) matching community expectations and recovery outcomes, with contrasting views between PLWP, who expressed hope, and family caregivers and providers, who expressed doubts and resignation; and (4) stigma as a cross-cutting barrier towards care and recovery across family-, community- and provider-levels linked to social exclusion among PLWP and family caregivers. Conclusion: Effective community programs must respond to local definitions of recovery, incorporate biomedical, traditional, and spiritual approaches, involve PLWP and family caregivers, and address stigma. Engaging Indigenous peoples’ practices in programs for PLWP can improve their acceptability, reach, and effectiveness.

Biodegradable and injectable curcumin-loaded hydrogel for the prevention of postoperative intrauterine adhesion

Qingmei Yang, Han Xu, Yang Chen et al.

Intrauterine adhesions (IUAs) are essentially fibrosis of the endometrium within the uterine cavity. It is a common cause of infertility in women and seriously affects their physical and mental health. Current therapeutic strategies have failed to reach satisfactory outcomes. Injectable and self-healing uterine hydrogels with antifibrotic properties would be efficient in preventing IUA. In this work, we prepared curcumin-loaded carboxymethyl chitosan (CMC)–oxidized hyaluronic acid (OHA) intrauterine hydrogel (Cur@CMC-OHA hydrogel) with antifibrotic properties, and its injectable and self-healing properties could be adapted to the morphostructures of the uterine cavity. The hydrogels exhibited tissue adhesive power, which is ideal for stable uterine cavity retention and therapeutic outcomes. In vivo experiments showed that in situ injection of the Cur@CMC-OHA hydrogel into a mouse model of IUA reduced fibrotic tissues, prevented IUA, and improved the reproductive outcomes. It effectively downregulated fibrosis-associated transforming growth factor-β1 (TGF-β1) expression and reversed epithelial–mesenchymal transition (EMT), resulting in anti-fibrotic and fertility restoration. In conclusion, Cur@CMC-OHA hydrogel may be a promising alternative for clinical treatment of uterine adhesion.

DNA methylation links between depression and immunity

Marisol Herrera-Rivero, Karina Gutiérrez-Fragoso, Monika Stoll et al.

DNA methylation is a reversible epigenetic mechanism of transcriptional regulation. Changes in DNA methylation patterns are influenced by internal and external factors, and have been associated with various human diseases. We hypothesized that specific methylation changes occur in depressed individuals, and that these changes are associated with disease progression and responses to pharmacological treatments. To test our hypothesis, we analyzed DNA methylation-wide changes in the BiDirect Study's depression and control cohorts. Over the course of two consecutive follow-up visits in 3-year intervals from baseline (i.e. 3 and 6 years after initial assessment), we identified a set of changes in methylation sites within gene enhancer and promoter regions that consistently changed only in depression patients or only in control individuals, respectively. Longitudinal changes in these methylation sites correlated with changes in circulating markers of inflammation and were enriched in biological processes related to innate and adaptive immunity. Currently, we are starting to explore the translational value of these findings by investigating their relationships with depressive symptoms and immunity. Moreover, by applying different machine learning algorithms, we test the utility of a prioritized subset of the identified methylation sites to predict disease status, disease progression, chronic inflammation status and antidepressant response. Overall, we expect to leverage longitudinal DNA methylation information to gain insights into the role of immunity in depression and generate computational tools with potential applications in the clinic.

Relationships Between Erectile Dysfunction, Depression, Anxiety, and Quality of Life in Young Japanese Men: A Cross-Sectional Study

Junichi Saito, Hiroaki Kumano, Mohammad Ghazizadeh et al.

Background: It is generally assumed that erectile dysfunction (ED) under the age of 40 is primarily driven by psychosocial factors. Aims This cross-sectional study evaluated ED, depression, anxiety, and quality of life among young Japanese men. Materials and Methods: We used an online survey to gather data from a wide range of community samples. ED was assessed by the IIEF-5 questionnaire, while depression, anxiety, and quality of life were assessed by the PHQ-9, GAD-7, and LISAT-8 questionnaires, respectively. Categorical data were evaluated by chi-square test and numerical data were estimated by one-way ANOVA. Post-hoc tests were conducted using the Holm method, which was used to control for type I errors. Cohen’s d index was calculated as effect sizes. Results: Valid responses from 329 individuals (age 33.93 ± 6.41 years) were obtained. According to IIEF-5 scores, the distribution of various severities of ED was as follows: non-ED, 37.39%; mild ED, 18.24%; mild to moderate ED, 27.36%; and moderate and severe ED, 17.02%. There were no significant differences in depression and anxiety between non-ED and ED men. In contrast, there were significant differences in quality of life between non-ED and ED men ( P < .01). Conclusions: We conclude that psychosocial factors may also be the cause of ED in young Japanese men besides depression and anxiety, and ED may reduce quality of life in various aspects. It is necessary to develop a Japanese version scale to measure ED-related psychosocial factors.

Whole body Hyperthermia for the treatment of depression

Martin Schäfer, Nils Auen, Florian Rübener et al.

Approximately 30% of patients with depressive episodes do not respond to conventional antidepressant medication or develop side effects. As a result, effective new treatments are urgently needed. Mood-enhancing effects of Whole body hyperthermia (WBH) have repeatedly been observed in various fields of medicine (e.g. rheumatology). A recent study by Janssen and colleagues (2016) with a randomized, double-blind and sham-controlled approach, was able to demonstrate a significant, rapid and partially lasting reduction of depressive symptoms in unmedicated patients with MDD following a WBH single session. Two randomized, double-blind, and (sham-) controlled clinical studies are currently conducted by our group in Essen (Germany). Overall, the studies focus on safety and tolerability data and the question whether WBH improves depressive symptoms in patients without any antidepressant treatment (HYPE I trial) or in patients with stable antidepressant treatment but non- or only partial response (HYPE II trial). In each subgroup, patients are randomized to either a verum group with two treatments with WBH in two weeks or a waiting group as controls. Beside response data, immune parameters in the serum of patients will be assessed.Preliminary results regarding safety and tolerability from the first 34 participants (N=16 treatment naive, N=18 non- or partly responder) showed, that WBH was well tolerated in most patients with or without antidepressant treatment. No serious side effects of WBH were observed that would have required further treatment. Overall, the risk for adverse effects during WBH was increased in case of somatic comorbidities. In summary, whole-body hyperthermia appears to be an interesting concept for a non-pharmacological treatment of perhaps subgroups like stress or immune induced depression.

Attitudes and Opinions of the Teaching Faculty Toward the LGBT Community

G. P. Gururaj, J. Shivananda Manohar, T. S. Sathyanarayana Rao

Studies have shown physician homophobia as a barrier to health care, with LGBTQIA+ patients being more likely to delay or avoid getting care due to fear of harassment or discrimination. Objectives: Our aims and objective in this study were to determine the attitudes and opinions of the various medical and surgical teaching faculties and compare them for gender differences, if any, toward the LGBT community. Methodology: It was a cross-sectional study done among 129 teaching faculty between November and December 2019 at Sri Devaraj Urs Academy of Higher Education and Research, a rural tertiary medical college in southern India with regular and ongoing postgraduate training in all medical and surgical specialties, with assessment done using a structured and validated instrument, Riddle homophobia scale, meant for the assessment of attitudes and beliefs toward the LGBT community. Results: Most of the teachers in both the medical and surgical groups reported favorable positive attitudes/beliefs with statements like homophobia and heterosexism are wrong, and there is no need for the LGBT people to undergo reparative surgery; they deserve the same rights and privileges as everyone; however, there were negative attitudes and opinions reported with the statements like the LGBT people did not choose their sexual orientation and teachers themselves need to undergo introspection to be able to be supportive toward them, which is a significant source of concern. There were no differences in attitudes and opinions based on the age groups of the teachers of both the groups. Male teachers, when compared to female teachers (male:females—84:45), reported more favorable beliefs ( t 2 / p value= 6.316/.0043) toward the LGBT community at p value ≤ .05. Conclusions: The teaching of sexuality, especially on the LGBT community, to medical professionals, especially teaching faculty, needs to be perceptive to the issues faced by people with different sexual orientations and identities.

Looking beyond the individual–The importance of accessing health and cultural services for Indigenous women in Thunder Bay, Ontario

Jonathan C. Lin, Elaine Toombs, Chris Sanders et al.

Access to cultural activities and culturally relevant healthcare has always been significant for achieving holistic Indigenous health and continues to be a key factor in shaping the health journey of Indigenous individuals and communities. Previous research has indicated the importance of cultural practices and services in sustaining cultural identity for Indigenous peoples, which is a major influence on their wellbeing. This study marks the first phase in a project aimed at establishing an Indigenous healing program and uses a qualitative research approach to understand the health and cultural services that Indigenous women want and require in Thunder Bay, Ontario. During interviews, participants (n = 22) answered questions around their understandings of health and wellbeing, and how they are able to incorporate cultural practices into their circle of care. Thematic analysis was performed on interview transcripts, and 4 key themes were identified: ‘independence and self-care’, ‘external barriers to accessing services’, ‘finding comfort in the familiar’ and ‘sense of community’. Together these themes illustrate how Indigenous women feel a strong sense of personal responsibility for maintaining their health despite the multiple environmental factors that may act as barriers or supports. Furthermore, the necessity of embedding cultural practices into Indigenous women’s circle of care is highlighted by the participants as they describe the mental, spiritual, social, and emotional health benefits of engaging in cultural activities within their community. The findings demonstrate the need for current modes of care to look beyond the individual and consider the impacts that socio-environmental factors have on Indigenous women. To accomplish this, we hope to increase access to health and cultural services through the creation of an Indigenous healing program that can be adequately incorporated into Indigenous women’s circle of care if they wish to do so.

Contextual adaptation and piloting of Group Integrative Adapt Therapy (IAT-G) amongst Rohingya refugees living in Bangladesh

Mahmuda Mahmuda, Mohammad Abdul Awal Miah, Mohamed Elshazly et al.

A main gap in the delivery of mental health and psychosocial (MHPSS) services to Rohingya refugees in Cox’s Bazar district in Bangladesh is the limited coverage of psychotherapeutic interventions. There is a shortage of mental health professionals who can provide culturally adapted and linguistically appropriate psychotherapeutic interventions that are congruent with the refugee experience. This article describes preliminary work on training Bangladeshi psychologists and para-professionals in applying a novel group-based psychological intervention for refugees, Group Integrative Adapt Therapy (IAT-G). This is a pragmatic, eclectic and transdiagnostic approach, based on the Adaptation and Development After Persecution and Trauma (ADAPT) model. It aims to help refugees to develop resilience and capacities for managing maladaptive reactions to trauma and post-migration living difficulties. This paper describes the key strategies and challenges during the first phase of the project. Activities included an extensive desk review, a one-week training workshop with competency assessments, cultural adaptation of the treatment manual and assessment measures, training on digital literacy and electronic data collection and a blended supervisory model (involving onsite and online sessions). It proved to be generally feasible but not easy to introduce IAT-G in this population. Key implications for practice The introduction of a brief scalable psychological intervention within an acute and ongoing emergency was feasible, but faced considerable challenges including high staff turnover. Implementation involves multiple components including stakeholder engagement, contextual adaptation of tools and materials, training and supervision and setting up a monitoring system. Setting up a supervision is of critical importance but can be complicated in the context of an unfolding emergency.
