Sexual Fantasies and Their Role in Sexual Satisfaction and Quality of Life Among Men with Sexual Dysfunction

Background: This study examines the relationship between sexual fantasies, sexual dysfunction (SD), and sexual well-being, focusing on individuals with SD group compared to healthy controls. Methods: Participants included patients with sexual health concerns diagnosed with erectile dysfunction (ED) or premature ejaculation (PME) and healthy controls. Assessment tools included the Arizona Sexual Experience Scale (ASEX-M), Wilson’s Sex Fantasy Questionnaire (WSFQ), sexual quality of life (SQoL) scale, and subjective sexual satisfaction (SSS) scale. Data were analysed using descriptive statistics, correlation coefficients, and two-way ANOVA tests. Results: Socio-demographic profiles of the cases and controls were matched for age, marital status, sexual activity, education, and religion, with a mean age of 30 years. With regards to WSFQ scores, no significant difference in total fantasy scores between the SD group and controls was found. Significant differences in intimate and impersonal fantasy domains were present. The SD group scored significantly poorer than controls in sexual satisfaction, ASEX, and SQoL. The correlation studies showed positive correlations between WSFQ and SSS, WSFQ and SQoL, and a negative correlation between WSFQ and ASEX in controls. No significant correlations in the SD group. ANOVA Studies revealed significant main effects of SD on SSS (F = 165.38, η 2 = 0.465, P = .001 variance), SQoL (F = 84.651, P < .001, η 2 = 0.308), and ASEX (F = 170.566, P < .001, η 2 = 0.473). Sexual fantasies also contributed to the variance in SSS (F = 4.592, P = .001, η 2 = 0.088), and SQoL (F = 3.746, P = .006, η 2 = 0.073). Conclusions: This study highlights the differential impact of sexual fantasies on individuals with and without SD. While fantasies positively affect sexual satisfaction and quality of life in healthy individuals, this effect is diminished in those with SD. The findings emphasize the need for a nuanced approach to addressing sexual fantasies in clinical practice, particularly for patients with SD.

Gambaran Parental Burnout pada Orang Tua dengan Anak Autism Spectrum Disorder

Penelitian ini bertujuan untuk mendeskripsikan gambaran parental burnout pada orang tua dengan anak autism spectrum disorder. Metode penelitian yang digunakan adalah penelitian kualitatif studi kasus intrinsik melalui wawancara, field notes, dokumen, dan artefak. Data dianalisis dengan analisis tematik oleh Braun & Clark. Hasil penelitian menunjukkan bahwa keseluruhan partisipan mengalami parental burnout yang unik, namun hampir serupa. Pengalaman parental burnout tersebut berupa pengalaman kelelahan, pengalaman ketidakmampuan, pengalaman kontras, pengalaman keberjarakan dengan anak, dan kesulitan-kesulitan selama pandemi Covid-19. Parental burnout yang terjadi disebabkan oleh faktor risiko dan protektif yang tidak seimbang. Faktor risiko tersebut di antaranya faktor risiko yang berasal dari internal individu, internal keluarga, dan eksternal. Sedangkan faktor protektif berasal dari dukungan dan rasa cinta pada anak. Hal tersebut berdampak pada diri sendiri, keluarga, anak, dan masyarakat. Untuk mengatasi parental burnout, dilakukan upaya-upaya yang berupa relaksasi, pembelajaran mandiri, membangun relasi dengan lingkungan, dan mengefektifkan parenting untuk mengatasi perilaku maladaptif anak.

Healthcare providers as patients: COVID-19 experience

There is compelling evidence for the psychological effects of the COVID-19 pandemic and earlier epidemics. However, fewer studies have examined the subjective meaning experience of healthcare providers who have survived COVID-19 as patients. This qualitative study aimed to understand further and describe the life experiences of healthcare providers who have survived COVID-19 as patients in Saudi Arabia. Data was collected using unstructured in-depth individual interviews among n = 10 healthcare providers from public hospitals in Saudi Arabia. Data were analyzed based on a phenomenological approach, which resulted in five themes: (i) physical and psychological signs and symptoms; (ii) self-healing, hiding pain, and family; (iii) fear of complications; (iv) disease stigma & long-term psychological outcomes; (v) emotional support, mental well-being & resignation. The overall synthesis showed that healthcare providers, as patients, experience the same difficulties and stressors as the general public. In some cases, these factors are even worse, as family members, colleagues, and employers develop a new type of stigma. Given the impact of social media and the flow of information of any type, more research is needed to examine the sources used to obtain information by the general public, whether these sources are reliable, and how the public can be taught to use only scientific data and not social data. Understanding the experience of healthcare providers as patients during the pandemic has allowed to look at the feelings and needs of people during illness from a new perspective. As expressed by participants, being a healthcare provider does not reduce the fear of the disease and does not mitigate its consequences in the form of stigmatization and isolation.
Neural correlates of PTSD in women with childhood sexual abuse with and without PTSD and response to paroxetine treatment: A placebo-controlled, double-blind trial

Objective: Childhood sexual abuse is the leading cause of posttraumatic stress disorder (PTSD) in women, and is a prominent cause of morbidity and loss of function for which limited treatments are available. Understanding the neurobiology of treatment response is important for developing new treatments. The purpose of this study was to assess neural correlates of personalized traumatic memories in women with childhood sexual abuse with and without PTSD, and to assess response to treatment. Methods: Women with childhood sexual abuse with (N = 28) and without (N = 17) PTSD underwent brain imaging with High-Resolution Positron Emission Tomography scanning with radiolabeled water for brain blood flow measurements during exposure to personalized traumatic scripts and memory encoding tasks. Women with PTSD were randomized to paroxetine or placebo followed by three months of double-blind treatment and repeat imaging with the same protocol. Results: Women with PTSD showed decreases in areas involved in the Default Mode Network (DMN), a network of brain areas usually active when the brain is at rest, hippocampus and visual processing areas with exposure to traumatic scripts at baseline while women without PTSD showed increased activation in superior frontal gyrus and other areas (p < 0.005). Treatment of women with PTSD with paroxetine resulted in increased anterior cingulate activation and brain areas involved in the DMN and visual processing with scripts compared to placebo (p < 0.005). Conclusion: PTSD related to childhood sexual abuse in women is associated with alterations in brain areas involved in memory and the stress response and treatment with paroxetine results in modulation of these areas.
Child Mental Health and Bullying within the Exposure to Domestic Violence: Literature Review

The harmful consequences of domestic violence on children’s lives have been widely reported in the literature. However, the influences of exposure to domestic violence or witnessing violence in the family on the children’s experiences of being a bully or victimized of bullying have not been paid sufficient attention in child mental health field. A critical literature review was the method of this article. Electronic databases about the relationship between bullying behavior and mental health by recognizing the consequences of domestic violence on children’s lives were searched. Exposure to domestic violence might be associated with children’s problematic behavior. Also, children might be bullied by peers at schools and behave aggressively. This article aims to explore the relationships between experiencing domestic violence and bullying behavior and being peer victimization by focusing on child mental health and intervention efforts. Understanding how children witness violence and how school bullying might be linked with witnessing violence is a goal of the study. Bullying can be recognized as a result of domestic violence incidence and this can improve effective interventions for children’s mental health and overall well-being. Thus, a deeper understanding of how child mental health conditions might be interconnected with bullying behavior within complex and dynamic domestic violence cases was explored.
Pengaruh Psychological Contract dan Perceived Organizational Support terhadap Employee Safety Behavior pada Karyawan

Pandemi Covid-19 menyebabkan kesehatan dan keselamatan karyawan yang bekerja secara langsung di kantor lebih berbahaya dibandingkan karyawan yang bekerja dari rumah. Hal tersebut karena adanya beberapa klaster penyebaran Covid-19 di beberapa perusahaan. Penelitian ini bertujuan untuk mengetahui bagaimana pengaruh psychological contract dan perceived organizational support terhadap employee safety behavior pada karyawan. Penelitian ini dilakukan pada 218 karyawan dengan usia 18-57 tahun yangs sedang bekerja secara langsung di kantor. Alat pengumpulan data berupa kuesioner Employee Safety Behavior, Psychological Contract Inventory, dan Perceived Organizational Support.Hasil penelitian dengan analisis regresi berganda menunjukkan bahwa psychological contract dan perceived organizational support secara bersamaan mempengaruhi employee safety behavior sebesar 27,9%. Psychological contract mampu memprediksi employee safety behavior sebesar nilai slope 0,1522. Perceived organizational support mampu memprediksi employee safety behavior sebesar nilai slope 0,0835. Tiga tipe psychological contract, yaitu relational, balance, dan transactional juga mampu memprediksi employee safety behavior.

Mental disorder or emotional distress? How psychiatric surveys in Afghanistan ignore the role of gender, culture and context

Over the last decades, mental health surveys in Afghanistan found very high prevalence figures for mental health problems among the Afghans. These epidemiological data suggest that the majority of the Afghan population suffer from a mental disorder such as depression or post-traumatic stress disorder. Such findings are often met with surprise by the Afghans who doubt that most of the people around them would suffer from a psychiatric illness. This paper explores the discrepancy between the findings from surveys using brief symptom-based questionnaires and the lived reality of the Afghan people. The authors argue that the outcomes of such mental health surveys should be interpreted with caution and can be better seen as indicators of ‘non-disordered’ psychosocial distress rather than as a general mental disorder. To better understand psychosocial wellbeing of the Afghan people, the survey data need to be put into context and have an eye for the cultural and social ecologies in which symptoms are produced. Many symptoms may actually be normal responses to living in difficult circumstances. Moreover, mental health surveys may conflate cultural idioms of distress with mental illness and often do not take into consideration that the Afghan social world is highly gender segregated. Future mental health research in Afghanistan should use contextually appropriate and culturally validated instruments and be complemented by in-depth ethnographic explorations of emotional suffering among Afghans.
Key implications for practice MHPSS practitioners should critically examine the results of psychiatric epidemiological surveys in humanitarian emergencies. Mental health researchers need to go beyond estimating prevalence figures of discrete mental disorders and move towards using dimensional approaches to mental health. Qualitative research can provide important insights around cultural idioms of distress that may be gender-specific.

Analysis of Family Clinical, vision of service nurses

Objective to know the practice of the Family Clinic in Cuiaba and its relationship with the precepts of the expanded clinic, from the perspective of the service nurses. Method qualitative descriptive research, data collection with semi-structured interviews and results analyzed according to the method of thematic content analysis. Results for nurses working in the service, this assumes a differentiated and innovative proposal, which seeks to correlate with shared management in its three spheres. Although most do not know the Enlarged Clinic term in his speech cited its main principles and its tools. The greatest potential described were related to the Support Center for Health and popular participation, and as challenges, the lack of community health worker, the national health establishment registration and the difficulty of operation due to the profile of the professionals technical level arising from secondary care. Conclusions The clinic has positive aspects that can contribute to the advancement of the profession, to train health professionals and an innovative primary care model. Therefore, it emphasizes the need for implementation of continuing education in order to realize its proposal, and further studies on site.

Urinary incontinence nursing diagnoses in patients with stroke

Abstract OBJECTIVE Identifying the prevalence of Stress urinary incontinence (SUI), Urge urinary incontinence (UUI), Functional urinary incontinence (FUI), Overflow urinary incontinence (OUI) and Reflex urinary incontinence (RUI) nursing diagnoses and their defining characteristics in stroke patients. METHOD A cross-sectional study with 156 patients treated in a neurological clinic. Data were collected through interviews and forwarded to nurses for diagnostic inference. RESULTS 92.3% of the patients had at least one of the studied diagnoses; OUI showed the highest prevalence (72.4%), followed by FUI (53.2%), RUI (50.0%), UUI (41.0%) and SUI (37.8%). Overdistended bladder and reports of inability to reach the toilet in time to avoid urine loss were the most prevalent defining characteristics. A statistically significant association of the defining characteristics with the studied diagnosis was verified. CONCLUSION The five incontinence diagnoses were identified in the evaluated patients, with different prevalence.

Instrument for assessing the quality of mobile emergency pre-hospital care: content validation

OBJECTIVES To validate an instrument to assess quality of mobile emergency pre-hospital care. METHOD A methodological study where 20 professionals gave their opinions on the items of the proposed instrument. The analysis was performed using Kappa test (K) and Content Validity Index (CVI), considering K> 0.80 and CVI ≥ 0.80. RESULTS Three items were excluded from the instrument: Professional Compensation; Job Satisfaction and Services Performed. Items that obtained adequate K and CVI indexes and remained in the instrument were: ambulance conservation status; physical structure; comfort in the ambulance; availability of material resources; user/staff safety; continuous learning; safety demonstrated by the team; access; welcoming; humanization; response time; costumer privacy; guidelines on care; relationship between professionals and costumers; opportunity for costumers to make complaints and multiprofessional conjunction/actuation. CONCLUSION The instrument to assess quality of care has been validated and may contribute to the evaluation of pre-hospital care in mobile emergency services.

Pesquisa-ação que objetivou estabelecer os parâmetros de avaliação clínica necessários ao paciente submetido à monitorização hemodinâmica pelo Cateter de Artéria Pulmonar (CAP) e construir um Protocolo de Cuidados de Enfermagem ao paciente grave e de risco em uso do cateter com os enfermeiros da UTI geral de um hospital público de Santa Catarina. A população se constituiu em uma amostra não probabilística intencional de 5 enfermeiros e utilizou para coleta de dados reuniões de grupo e questionário. O estudo é apresentado mediante estatística descritiva e análises qualitativas das questões subjetivas. Conclui-se que os critérios de avaliação clínica necessários ao paciente submetido à monitorização hemodinâmica se constituem em parâmetros invasivos e não invasivos e que o protocolo fundamenta a tomada de decisão clínica para o cuidado do paciente em uso do Cateter de Artéria Pulmonar.<br>Se trata de una Investigación-acción que tuvo como objetivo establecer los parámetros de evaluación clínica necesarios para el paciente sometido a monitorización hemodinámica por el Catéter de Arteria Pulmonar (CAP) y construir un Protocolo de Cuidados de Enfermería para el paciente grave y de riesgo en el uso del catéter, con los enfermeros de una Unidad de Cuidados Intensivos (UCI) general de un hospital público de Santa Catarina. La población se constituyó en una muestra no probabilística intencional de 5 enfermeros, utilizándose para la recolección de datos reuniones de grupo y un cuestionario. El estudio se presenta por medio de estadística descriptiva y de análisis cualitativo de las preguntas subjetivas. Se concluye que los criterios de evaluación clínica necesarios para el paciente sometido a monitorización hemodinámica se constituyen en parámetros invasivos y no invasivos y que el protocolo fundamenta la toma de decisión clínica para el cuidado del paciente que usa el CAP.<br>This research-action type study aimed to establish clinical evaluation parameters essential to the patient subject to hemodynamic monitoring by Pulmonary Artery Catheter (PAC) and to develop a Nursing Care Protocol to critical care patients using catheter with the nurses of an ICU in Santa Catarina State. The population consisted of a non-probabilistic intentional sample of five nurses, and the data were collected through group meetings and questionnaire. The study is presented with descriptive statistics and qualitative analyses of the subjective questions. In conclusion, the clinical evaluation criteria, essential to the patient subjected to hemodynamic monitoring, are constituted by invasive and non-invasive parameters and the protocol grounds making clinical decisions when caring for patient using PAC.

O estudo investigou os conflitos vivenciados pelas adolescentes com a descoberta da gravidez. A pesquisa descritiva com 12 adolescentes grávidas foi realizada em Jucás-Ceará. Os achados foram analisados após agrupamento em categorias temáticas emergidas das falas das entrevistadas ao responderem à indagação: Como tem vivenciado a gravidez na adolescência? Verificou-se que a gravidez era vista como um problema indesejado e que as adolescentes tinham medo de partilhar sua descoberta com a família ou o companheiro. Foram observadas reações dos pais ou responsáveis e o baixo nível socioeconômico como determinantes da não aceitação da gestação. Concluiu-se que a gravidez na adolescência constitui-se um problema de Saúde Pública, que deve ser visualizado amplamente, percebendo-se a adolescente e seus problemas cotidianos.<br>En el estudio se investigó los conflictos vivenciados por las adolescentes al descubrir su embarazo. Esta investigación descriptiva realizada con 12 adolescentes embarazadas se llevó a cabo en Jucás-Ceará. Los hallazgos fueron analizados después del agrupamiento en categorías temáticas que emergieron de los discursos de las entrevistadas al responder a la pregunta: Cómo has vivenciado el embarazo en la adolescencia? Se verificó que el embarazo era visto como un problema indeseado y que las adolescentes tenían miedo de compartir su descubrimiento con la familia o el compañero. Fueron observadas reacciones de los padres o responsables y el bajo nivel socioeconómico como determinantes de la no aceptación de la gestación. Se concluyó que el embarazo en la adolescencia se constituye en un problema de Salud Pública, que debe ser visualizado ampliamente, percibiéndose a la adolescente y sus problemas cotidianos.<br>This descriptive research with 12 pregnant teenagers was conducted in Jucás, State of Ceará. The findings were analyzed after being grouped in the thematic categories that emerged from the interviews carried out with the young women, who were asked to answer the question: How have you been experiencing pregnancy in adolescence?It was verified that pregnancy was seen as an undesired problem and that the pregnant teenagers were afraid to share their state with their family or partner. It was observed that the reaction from parents or legal guardians and the low socio-economic level were determinant for the non-acceptance of pregnancy. It was concluded that pregnancy in adolescence is an issue that belong to the realm of Public Health, and that it must be seen as part of a wider picture, in which the adolescents and their daily problems need to be considered.

Partos domiciliares planejados assistidos por enfermeiras obstétricas: transferências maternas e neonatais

O presente trabalho trata-se de estudo exploratório-descritivo que teve como objetivo descrever a taxa e as causas de transferência intraparto para o hospital de mulheres assistidas no domicílio por enfermeiras obstétricas e os desfechos desses nascimentos. A amostra foi composta por onze mulheres e seus recém-nascidos, de janeiro de 2005 a dezembro de 2009. Os dados foram coleta-dos em prontuários e cadernetas de saúde e analisados por estatística descritiva. A taxa de transferência foi de 11%, a maioria de nulíparas (63,6%), e todas foram transferidas durante o primeiro período clínico do parto. Os motivos mais frequentes de transferência foram parada de dilatação cervical e progressão da apresentação fetal, e desproporção cefalopélvica. Os escores de Apgar no 1º e 5º minutos foram >7 em 81,8% dos casos e não houve internação em unidade de terapia intensiva neonatal. Neste estudo constatou-se que o parto domiciliar planejado assistido por enfermeiras obstétricas, com protocolo assistencial, apresentou bons resultados maternos e neonatais, mesmo quando a transferência para o hospital foi necessária.

A maioria dos trabalhos publicados até o momento sobre assistência religiosa de enfermagem dizem respeito às práticas religiosas das crenças mais comuns em nosso meio. Após ligeira conceituação sobre a necessidade religiosa, a autora cita algumas bases para a assistência de enfermagem adequada neste campo, que servem de orientação tanto ao profissional como ao estudante. Estes aspectos incluem: aceitação do individuo e sua religião, ambiente favorável para praticá-la e conhecimentos dos princípios que regem as práticas. A implantação e o desenvolvimento da fé no paciente, pelo exemplo da enfermeira, também são aspectos abordados.

Desarrollo y validación de un instrumento para la evaluación inicial de enfermeira

Este estudio, desarrollado de abril a julio de 2008, objetiva desarrollar y validar un instrumento para orientar la evaluación de enfermería en parámetros asistenciales y docentes en Santa Cruz, Bolivia. Participaron doce enfermeras bolivianas y se utilizaron el análisis de documentos y técnicas de consenso para definir las categorías y criterios a ser incluidas en el instrumento. Estas categorías son: la evaluación física y los 11 Patrones Funcionales de Salud de Gordon. El instrumento se caracteriza por ser breve, de sencilla comprensión y por centrarse en el enfermero. No incluye elementos de evaluación física avanzada, pero sí criterios de estilos de vida y autonomía del paciente. El desarrollo de estos instrumentos contribuye a la calidad de los registros, favorece el juicio clínico y la aplicación del proceso a la enfermería, refuerza el papel del enfermero, favoreciendo la sistematización de su práctica.

Suicídio: um ensaio em busca de um des-velamento do tema Suicide: one essay in a search of a revealment of the theme

Este ensaio propõe-se a des-velar facetas do tema suicídio por meio do discurso de diferentes autores que tratam do tema, bem como mediante os contatos que pude ter, durante os estágios de aprendizado, coin pessoas que tentaram suicídio. Minha trajetória acadêmica inclui uma incursão pelas idéias da fenomnenologia, particularmente pelos pensamentos de Heidegger, ou seja, análise existencial do homem enquanto ser-aí. Nesse sentido, a compreensão da pessoa que decide colocar fim à sua existência, pode se constituir, na perspectiva desta análise, como um caminho para reconstrução e redimensionamento de suas perspectivas existenciais.<br>This essay proposes to reveal facets of the suicide through the discourse of differents authors treating this theme as well as through contacts that I was able to have in my nursing training, through my academic trajectory. This trajectory includes an incursion by phenomenological ideas, mainly by the ideas of Heidegger and his existencial analysis of the man as being-there.
