Innovations to improve outcomes and uptake of psychotherapies for mental disorders: a state‐of‐the‐art review

Psychotherapies have been found effective in the treatment of most mental disorders. However, substantial improvements are still much needed, and many innovations of therapies are currently being developed. We review the current status of promising innovations to improve the outcomes and uptake of psychotherapies for mental disorders, discussing the largest and most recent meta‐analyses. Innovations are categorized into four domains: a) the digital field (including Internet‐based interventions in general; mobile interventions; serious games; virtual and augmented reality; prescription digital therapeutics; blended therapy; avatar therapy; and chatbots/artificial intelligence‐generated conversational agents); b) personalized treatments (research on predictors and moderators in large randomized controlled trials; use of individual patient data meta‐analyses in personalization; machine learning approaches; personalized and modular therapies; and matching therapists to patients); c) new and improved therapies (cognitive bias modification; cognitive remediation; psychedelic‐assisted psychotherapies; transdiagnostic therapies; research on effective components through factorial trials and component network meta‐analyses; innovations in the understanding of the processes involved in psychotherapies, including research on common factors and the therapeutic alliance, and on the fidelity vs. flexibility question; research on prevention of adverse effects of therapies, the impact of increased session frequency or progress feedback on outcomes, and methodological innovations in trial designs); and d) dissemination and simplification of therapies (task sharing, digital interventions in low‐ and middle‐income countries; and single‐session interventions). These innovations vary in their maturity, from dozens of supporting trials to few or none. Methods to assess the strength of innovations suggest that no innovation will be a paradigm‐shifting “silver bullet” that dramatically increases treatment outcomes, but that progress will only be possible through multiple, incremental improvements.

Effectiveness of Web-based Parenting Intervention in Improving Mindful Parenting among Parents of Elementary School Aged Children

The complexity of parenting in the current digital era requires web-based intervention innovation to provide attentive care that impacts the quality of care. This study examined the effectiveness of a web-based parenting intervention in improving mindful parenting among parents of elementary school–aged children. Fifty-seven participants joined the intervention program with parent-population characteristics (fathers and mothers) living with children aged between 7 and 12 years old. The instrument consisted of pretest-posttest mindful parenting scale. This quasi-experimental research used one group pretest-posttest group design. The results indicate a substantial enhancement in mindful parenting scores post-intervention (Z = −3.544; p = 0.000), with the average score increasing from 2.735 to 2.889. A Cohen’s d of 0.44 indicates a moderate effect size with meaningful practical significance. Dimension-level analyses reveal significant improvements in three dimensions listening with empathy, understanding and acceptance, and self-regulation. In the other two dimensions, namely emotional regulation and compassion, did not show significant changes. The understanding and acceptance dimension exhibits the greatest impact, with the highest mean increase and the largest effect size (d = 0.638). These findings indicate that the web-based intervention effectively enhances the cognitive aspects of mindful parenting but remains less optimal in addressing affective components that require more intensive emotional practice. This study offers important implications for the development of digital parenting programs and provide directions for future intervention research.

“La sirena llamo. El monstruo respondió”

El texto se ocupa del cuento de Ray Bradbury, “La sirena en la niebla” [“The Fog Horn”], para indicar de qué manera el sonido de un faro se transforma en llamado para el último dinosaurio de mar. El encuentro de los personajes con la bestia remota termina por ser una despedida, muy a la manera del amor cortés, enarbolando el estandarte de lo imposible. Ese adiós queda como emblema de la destrucción de una de las modalidades esenciales de la alteridad para el ser humano: la alteridad animal. Siguiendo las líneas del cuento, queda, además, planteado el interrogante mayor relativo a la vida en el tiempo.

Balneo physio gynecology in Bihor County

Introduction: The beneficial effects of balneotherapy on gynecological conditions are due to the physical-chemical properties of thermo-mineral waters, through external and internal applications. Objective: This study highlights the localities in Bihor County that have hydro- mineral resources with physio-chemical properties, aimed at therapeutic indications in the management of gynecological pathologies. Methods: A bibliographic study was carried out on the effect of balneotherapy in gynecological conditions, preceded by the identification, selection and centralization of the localities in Bihor County that have balneary resources valued in gynecological pathologies. Results: Out of the total of 81 localities with hydromineral resources found in Bihor County, the hydromineral resource is exploited for balneotherapeutic purposes only in six localities (Băile Felix, Băile 1 Mai, Băile Tinca, Sarcău, Marghita, Ștei), of which only four localities have resources hydrominerals with therapeutic indications aimed at gynecological conditions (Băile Felix, Băile 1 Mai, Marghita, Băile Tinca, Sarcău). Conclusions: The balneary treatment certainly has its own, primary, but also complementary role in the treatment of chronic gynecological diseases and is increasingly recommended. However, future studies would be necessary to refer to gynecological pathologies and the influence that the properties of thermal waters can have on them.

Gudu-Guduwa: Healing Disconnection Through Incorporating Bush and Animal-Assisted Therapies Into Therapeutic Practice: A First Nations Reflection

As mental health therapists, too often we see outward symptoms of lives impacted by complex trauma. Disconnection from self, others (families and community), and Country is at a crisis point. Looking to our First Nations healing knowledges, the author explores her experiences as a First Nations woman and therapist, and how she incorporates bush and animal assisted therapies into her private practice. Areas that the article touches on include healing on your ancestral Country, healing on Country that is not your own, healing when you do not know where your Country is, and incorporating animal assisted therapies into your healing modalities.

Empatía y personalidad como variables predictoras de la homofobia sutil y manifiesta

La discriminación por orientación sexual y la identidad de género sigue siendo un problema en la mayoría de los países. Este trabajo explora las dimensiones de la personalidad y los componentes cognitivos y afectivos de la empatía, con la finalidad de determinar el valor predictivo que tienen sobre las actitudes homofóbicas encubiertas y explícitas. La muestra estuvo formada por 849 estudiantes universitarios. Se utilizó la Escala de Homofobia Sutil y Manifiesta (EHMS), el Inventario de Personalidad (NEO-FFI) y el Interpersonal Reactivity Index (IRI). Los resultados muestran una tendencia hacia actitudes homófobas sutiles en los estudiantes universitarios. Además, tanto la empatía cognitiva como la afectiva y la apertura a la experiencia y la amabilidad se relacionan y predicen actitudes más favorables hacia la homosexualidad, pudiendo actuar como variables protectoras hacia el colectivo LGTBI y la diversidad sexual. Se destaca la importancia de intervenciones basadas en programas educativos y la transmisión de valores.

Can therapists be autistic?

This article explores a systemic therapist’s dual role as a family therapist and an autistic person, detailing her journey of integrating her autism diagnosis with her professional identity. The article explores autistic communication in reference to the double empathy problem (Milton 2012), the concept that communication difficulties between autistic and non-autistic individuals are due to mutual misunderstandings, and the importance of direct communication in working with autistic clients. It explores the stigma associated with autism and the considerations of disclosing one's autistic status in therapeutic and personal contexts, highlighting the importance of understanding and accommodating autism in therapeutic settings. Therapeutic approaches with autistic people are explored, such as the PPRR model (Burnham 2017). The paper advocates for post-diagnosis support and longer-term work with families, stressing the value autistic therapists bring to the profession by offering unique insights and validation to autistic clients.

Attachment in Supervision: Using a Relational Lens to Understand Supervisory Dynamics

This article aims to introduce a relational model of supervision based on attachment theory using the Circle of Security as a model for understanding supervisory dynamics. The article briefly reviews attachment theory and the historical context of clinical supervision and provides a brief discussion regarding the importance of ongoing supervision as a way to support frontline staff and mitigate the impact of secondary traumatic stress and burnout. Applying concepts from reflective supervision and attachment theory, the article introduces the Supervisor's Circle of Security and presents a matrix model of supervisor—supervisee relationships, lending 16 possible attachment combinations for the dyad. Potential presentations of each of the dyadic compositions is discussed followed by clinical vignettes.

Resiliensi Akademik Siswa Sekolah Menengah Pertama dalam Mengikuti Pembelajaran Tatap Muka Terbatas (TMT) di Daerah Khusus Ibukota (DKI) Jakarta

This study aims to identify the level of academic resilience of junior high school students (First Secondary School) during the period of Pembelajaran Tatap Muka Terbatas  (Limited Face-to-Face Learning) during the Covid-19 pandemic. The type of research using quantitative descriptive design with the population used is Muhammadiyah Junior High School 2 Jakarta (n=379). The sampling technique uses Convention sampling. This study used a questionnaire adapted from the Indonesian version of The Academic Resilience Scale-30 (ARS-30) with a lycert scale. The data obtained will be analyzed using categorization tests through the IBM SPSS Statistics 26 and Microsoft Excel applications. The results of the study showed that 311 students (82.1%) had a high academic resilience rate and 68 students (17.9%) had a moderate resilience rate determined through measuring academic resilience dimensions (1) Perseverance or persistence, (2) Reflecting and Adaptive Help Seeking, (c) Negatif Affect and Emotional Response. Therefore it can be inferred that the academic resilience rate of junior high school students in the Pembelajaran Tatap Muka Terbatas  period has a relatively high category.

Asimismo, se describe el tipo de obediencia que se desarrolla bajo esa cosmovisión política, y el sometimiento al que se entregan quienes se adhieren a ella. En todo ese conjunto, se afirma la existencia de un isomorfismo entre pandemia y mercado, que lleva a cuestionar por qué para el sujeto neoliberal no hay rebelión posible.

Investigating psychometric properties of Selfobject Needs Inventory in Turkish society

Selfobject Needs Inventory (SONI) is a scale developed to measure the orientation of an adult toward selfobject needs. The aim of this study was to investigate psychometric properties of SONI in Turkish society. The research was carried out on two samples. Principal Component Analysis and reliability analysis were performed on 391 subjects (305 women and 86 men) with the average age of participants as 38.9 years (SD = 6.32). Criterion- related validity indicators were investigated on 104 subjects (76 women and 28 men) with the average age of participants as 29.91 years (SD = 10.65). Principal Component Analysis results revealed 3-factor structure and factors were named as “approach orientation toward mirroring, idealization/twinship”, “avoidance orientation from idealization/twinship” and “avoidance orientation from mirroring”. Cronbach’s alphas for the factors were found as .84, .79, and .65, respectively; and item-total correlation coefficients ranged between .25 and .60. The relationships between SONI subscales, subscales of Scale of Dimensions of Interpersonal Relationships (SDIR) and subscales of Basic Personality Traits Inventory (BPTI) were used for concurrent validity.

System of relations of drug-dependent personality

The paper discusses the basic psychological directions of the system of attitudes of drug addiction’s study. The analysis of the problem of the system of attitudes of drug addiction persons made it possible to consider the relationship of drug addicts as a triad of subject-subject attitude to the drug, subject-object attitude to other people and subject-object arritude to himself. Thus, the drug "humanizes", endowed with attributes of a person's mental life. Emotional attachment to the drug is "personalized," and its loss is experienced as the loss of some part of itself. Behavior in relation to the drug is manifested in his persistent search. While, another person is treated by drug addicts in terms of utility, acceptability, need, importance, importance for them. The result of the analysis of different points of view about the peculiarities of the system of attitudes among drug addicts was the construction of a model for studying the relationship of persons with drug addiction in the triad: the subject is a subjective attitude to the drug, the subject is the object relation to other people, and the subject is an objective relation to himself. Drug addicts, animate and personalize the drug, refer to it as a person. At the same time, their relation to other people and to themselves is reified, and others are perceived as objects for manipulation. The findings suggest that formation of subject-subject emotional stereotypes towards close women: mother and wife/girl who are based on the depreciation mother’s social status and further in exaggerating the negative qualities of a partner.

ASPECTS ON MUSCULO-SKELETAL TRAUMAS IN COMPETITIVE SPORTSMEN. A COMPARATIVE STUDY BETWEEN SPORT BRANCHES

Aim. The aim of this study is to establish the incidence, frequency and location of musculo-skeletal trauma in the joints at competitive athletes, in a comparison between sport branches, age groups and time spent in training. Material and method: The study was performed on a batch of 155 sportsmen who practiced athletics, basketball, handball, football and volleyball. The sportsmen were between 13 -42 years old and had been practicing sports for 4-20 years. We recorded and compared the percentage of traumas in the group of athletes, depending on the affected segment, the age groups and the time spent in training, in two periods of time.The study covered three years of competitions, the Z test was applied and considered a significance level α = 0.05. Results. Comparing the percentages between basketball and handball players traumas, we obtained significance for the following segments. Spine, leg, knee, shoulder - traumas in basketball are significantly less (p=0.014, α=0.05) than in handball. Hand, palm, fist - traumas in basketball ocurring significantly often ( p<0.001, α=0.001) than in handball. between basketball and volleyball players Knee and shoulder – traumas in basketball are significantly less ( p<0.001, α=0.001) (p= 0.003, α =0.01) than in volleyball. between handball and volleyball players Hand, palm, fist - traumas in handball are significantly less (p<0.001, α=0.001) than in volleyball Conclusions: The performance level and the number of training sessions influenced the high number of traumas that occurred. The extrinsic factors have a strong influence.
