Women's health facility choices for antenatal, delivery, and postnatal care in Eastern Visayas, Philippines

Ahreum Choi, Heunghee Kim, Sherlyn Mae P. Provido et al.

BackgroundThis study aims to identify socioeconomic factors associated with the choice of antenatal care (ANC) facilities and to analyze trends in the utilization of health facilities for delivery and postnatal care (PNC) based on the type of ANC facility in Eastern Visayas, Philippines.MethodsThis secondary data analysis uses baseline and one-year follow-up survey data from a quasi-experimental study conducted in September 2023 and 2024. Data from 1,414 women with information on maternal health facility utilization was analyzed. ANC facilities were categorized into four groups: Barangay Health Station (BHS), Rural Health Unit (RHU), hospital/clinic and others. Multinomial logistic regressions were applied, adjusting for socio-economic status and Barangay location, to examine associations between socio-economic factors and ANC facility choice, as well as trends in delivery and PNC facility utilization based on ANC facility type.ResultsAmong 1,414 postpartum mothers, 35.6% received ANC at BHS, 34.1% at RHU, 32.7% at hospital/clinic, and 0.6% did not receive ANC. Most deliveries (83.3%) and PNC (61.4%) services occurred in hospital/clinic settings. Mothers who received ANC at a hospital/clinic were more likely to have higher education (aRRR = 7.04, 95% CI: 3.97, 12.50) and be wealthier (aRRR = 2.00, 95% CI: 1.09, 3.69) compared to those who received ANC at BHS. Mothers receiving ANC at RHU (aRRR = 0.52, 95% CI: 0.34, 0.79) or hospital/clinic (aRRR = 0.55, 95% CI: 0.38, 0.78) were less likely to be single with a partner compared to those receiving ANC at BHS. Mothers who received ANC at hospital/clinic were more likely to deliver at a hospital/clinic (aRRR = 8.49, 95% CI: 3.56, 20.26) than at a RHU/BHS, and to receive PNC at a hospital/clinic (aRRR = 2.07, 95% CI: 1.32, 3.24) instead of at a BHS, compared to those receiving ANC at BHS. Mothers receiving ANC at RHU were more likely to also receive PNC at an RHU (aRRR = 16.13, 95% CI: 7.80, 33.36) compared to those receiving ANC at BHS.ConclusionsSocioeconomic disparities are associated with ANC facility choice, which in turn affects subsequent decisions regarding facilities for delivery and PNC in Eastern Visayas. As such, facility selection should be guided by healthcare needs rather than socioeconomic status.

CATATYMNO-IMAGINATIVE PSYCHOTHERAPY IN THE TREATMENT OF PATIENTS WITH ANXIETY-PHOBIC DISORDERS

Yusupova Guzel

Modern conditions are not sufficient for a psychotherapist to study and apply only one modality in practice. Requires a level of professional competence that allows to use the psychotherapeutic potential of various psychotherapeutic modalities, in combining them in their work. In this case, the multimodal (integrative) approach of using the method of catatymno-imaginative symbolic drama allows fully implementing the principle of an individual approach to each patient, providing the opportunity to take into account not only the nature of his mental disorders, but also personal characteristics such as the level of intelligence, emotional state, and also behavior. This research aims to reveal the peculiarities of organizing and implementing psychotherapy using the catatym-imaginative drama symbol method involving 14 outpatients in various departments of the State Budget Agency who have been diagnosed with anxiety based on ICD-10. The testing method used in this research was the Sheehan Anxiety Scale and anxiety assessment techniques based on the Spielberger-Khanin theory. The measurements were carried out at the initial session of the meeting and the final session (8th meeting). The method used is combined with body-oriented psychotherapy. The results of this study showed that respondents with anxiety-phobic disorders had a positive correlation, and this relationship was more pronounced before training. Pearson correlation coefficient (r) was 0.87 (before training) and 0.79 (after training). It can be concluded that in the treatment of patients with anxiety-phobic disorders, the method of catatim-imaginative therapy significantly reduces the negative symptoms.

To be or not to be a mother: doubtful, fraught, and denied access to motherhood in contemporary Catalan theatre

Adriana Nicolau Jiménez

Like many other regions in Europe, Catalonia has seen significant changes in the demographics of motherhood over recent decades, with decreasing fertility rates and an increase in the average age of women at the birth of the first child. Together with increased social awareness of gender and identity issues such as gestational loss or the sexual and reproductive rights of people with disabilities, this changing landscape seems to have had a noteworthy impact on contemporary Catalan theatre. Indeed, throughout the long 2010s, a significant number of female-authored plays focused on motherhood premiered in Catalan venues, and a significant portion of these addressed doubtful, fraught, and denied access to motherhood. This essay analyses four important works associated with this trend: Júlia & The Empty Lot (2018) and To Be a Mother (2021) by Clàudia Cedó, Conversations With My Uterus and Other Interlocutors (2018) by Núria Planes Llull, and Stigmas (2019) by Concha Milla. These plays respectively address the experience of perinatal loss, the desire of women with disabilities to become mothers, doubts concerning one’s maternal desire, and the consequences of infertility and assisted reproductive technology processes. The analysis is based on dramatic and video criticism, accompanied by a study of performance documentation and paratexts. This essay contends that the analysed plays shed light on the tension between hegemonic narratives around motherhood and the actual experiences of contemporary Catalan women in terms of access to motherhood, which can be fraught with social, political, or physiological obstacles, as well as denied to certain subjects who have historically been excluded from womanhood, such as women with disabilities. In so doing, the plays expand the available narratives for what we understand as motherhood while elaborating feminist responses to the lived experiences they bring to the stage.

Bride Price in the Patriarchal South Sudanese Society: A Legal Perspective

Mark A W Deng

This article discusses bride price and numerous problems associated with it from a legal perspective. Two such problems, indeed, the most serious of all, are: (i) bride price objectifies and commodifies women, and this lowers women’s status in society; and (ii) bride price violates a woman’s freedom to choose who to marry as all but young girls, that is, those under the age of 18, are forced into marriage by their families for bride-wealth. Young girls who dare to resist can be tortured severely, or even put to death. These practices violate the Transitional Constitution of South Sudan 2011 and the Child Act 2008 under which ‘customs’ and ‘traditions’ that are harmful to women and children are explicitly prohibited. They are equally in conflict with regional and international human rights conventions that proscribe similar practices. In light of all these issues, the article recommends bride price to be abolished through legislative measures—i.e. for the Government of South Sudan to enact a law to criminalise bride price. While this suggestion is likely to be met with resistance, given how deeply culturally ingrained this practice is, what should be compelling for the government is securing women and children’s rights and freedoms.

SARS-CoV-2 in Pregnancy: Fitting Into the Existing Viral Repertoire

Roopali Rajput, Jitender Sharma

The risk of viral infection during pregnancy is well-documented; however, the intervention modalities that in practice enable maternal-fetal protection are restricted by limited understanding. This becomes all the more challenging during pandemics. During many different epidemic and pandemic viral outbreaks, worse outcomes (fetal abnormalities, mortality, preterm labor, etc.) seem to affect pregnant women than what has been evident when compared to non-pregnant women. The condition of pregnancy, which is widely understood as “immunosuppressed,” needs to be re-understood in terms of the way the immune system works during such a state. The immune system gets transformed to accommodate and facilitate fetal growth. The interference of such supportive conversion by viral infection and the risk of co-infection lead to adverse fetal outcomes. Hence, it is crucial to understand the risk and impact of potent viral infections likely to be encountered during pregnancy. In the present article, we review the effects imposed by previously established and recently emerging/re-emerging viral infections on maternal and fetal health. Such understanding is important in devising strategies for better preparedness and knowing the treatment options available to mitigate the relevant adverse outcomes.

Sompa and Dui Menre in Wedding Traditions of Bugis Society

Rusdaya Basri, Fikri Fikri

This paper investigates the dynamics of sompa and dui’ menre’ in wedding traditions of Bugis society in Sidenreng Rappang Regency, South Sulawesi. Sompa in Bugis society is giving money or property to the wife that becomes the pillar and the legal requirement in the wedding traditions. Then, dui’ menre’ is the money from the groom that must be dedicated to the bride’s family as the budget for the wedding procession. The high value of sompa and dui’ menre’ in Bugis wedding tradition is determined by the social status of the bride and her parents that consists of nobility, wealth, higher education and her beauty. In terms of Maqasid al-Sharia perspective, dui’ menre’ has a mashlah}at ’ sides that can motivate the youths to work hard to have income and as the symbols of noble position for the woman. However, that tradition also leads some mudharat sides; for examples, many men failed to marry; there is a tendency for sirri marriage (unregistered marriage), they do eloping and they tend to delay the age of marriage (preferring to be celibacy or spinster).

CONCEPTUAL REFERENDUM- SOCIO-LEGAL ASPECTS AND SOCIAL CONSEQUENCES

I. Apostu

The culture in which the individual is born and socialized will always impress upon him. Analyzing the surface of social polemics about the "danger" of LGBT marriage, heterosexuality seems to be the main landmark of marital cohabitation, and minority alternatives seem to be a form of symbolic aggression that most feel sympathetic to accepting. Also, looking from the perspective of family protection actions through this ample national action, the referendum becomes more of an action to protect a simple concept of marriage than to protect the marital conjugal space, around which all fears about marriage are being developed (with all the "risks" of being "veiled" by same-sex couples claims). However, the legal modification of marriage only between man and woman does not protect conjugality from the "danger" of legalizing homosexual marital unions, nor from marriage-like rights such as adoption. The study aims to analyze comparatively international laws on consensual union and marriage, insisting on socio-legal resources that express rights similar to marriage in the European space.
