Blockchain Technology for Healthcare: Facilitating the Transition to Patient-Driven Interoperability

Interoperability in healthcare has traditionally been focused around data exchange between business entities, for example, different hospital systems. However, there has been a recent push towards patient-driven interoperability, in which health data exchange is patient-mediated and patient-driven. Patient-centered interoperability, however, brings with it new challenges and requirements around security and privacy, technology, incentives, and governance that must be addressed for this type of data sharing to succeed at scale. In this paper, we look at how blockchain technology might facilitate this transition through five mechanisms: (1) digital access rules, (2) data aggregation, (3) data liquidity, (4) patient identity, and (5) data immutability. We then look at barriers to blockchain-enabled patient-driven interoperability, specifically clinical data transaction volume, privacy and security, patient engagement, and incentives. We conclude by noting that while patient-driving interoperability is an exciting trend in healthcare, given these challenges, it remains to be seen whether blockchain can facilitate the transition from institution-centric to patient-centric data sharing.

Blockchain Consensus Protocols in the Wild

A blockchain is a distributed ledger for recording transactions, maintained by many nodes without central authority through a distributed cryptographic protocol. All nodes validate the information to be appended to the blockchain, and a consensus protocol ensures that the nodes agree on a unique order in which entries are appended. Consensus protocols for tolerating Byzantine faults have received renewed attention because they also address blockchain systems. This work discusses the process of assessing and gaining confidence in the resilience of a consensus protocols exposed to faults and adversarial nodes. We advocate to follow the established practice in cryptography and computer security, relying on public reviews, detailed models, and formal proofs; the designers of several practical systems appear to be unaware of this. Moreover, we review the consensus protocols in some prominent permissioned blockchain platforms with respect to their fault models and resilience against attacks. The protocol comparison covers Hyperledger Fabric, Tendermint, Symbiont, R3~Corda, Iroha, Kadena, Chain, Quorum, MultiChain, Sawtooth Lake, Ripple, Stellar, and IOTA.

A Systematic Review of the Use of Blockchain in Healthcare

Blockchain technology enables a decentralized and distributed environment with no need for a central authority. Transactions are simultaneously secure and trustworthy due to the use of cryptographic principles. In recent years, blockchain technology has become very trendy and penetrated different domains, mostly due to the popularity of cryptocurrencies. One field where blockchain technology has tremendous potential is healthcare, due to the need for a more patient-centric approach to healthcare systems and to connect disparate systems and increase the accuracy of electronic healthcare records (EHRs). In this systematic review, an analysis of state-of-the-art blockchain research in the field of healthcare is conducted. The aim is to reveal the potential applications of the technology and to highlight the challenges and possible directions of blockchain research in healthcare. First, background information is discussed, followed by a description of the exact methodology used in this paper. Next, an analysis of the results is given, which includes a bibliometric overview, an analysis of gathered data and its properties, and the results of a literature quality assessment. Lastly, there is a discussion of the results from the analysis. The findings indicate that blockchain technology research in healthcare is increasing and it is mostly used for data sharing, managing health records and access control. Other scenarios are very rare. Most research is aimed at presenting novel structural designs in the form of frameworks, architectures or models. Findings also show that technical details about the used blockchain elements are not given in most of the analyzed publications and that most research does not present any prototype implementation or implementation details. Often even with a prototype implementation, no details about blockchain elements are given.

Blockchain Practices, Potentials, and Perspectives in Greening Supply Chains

Blockchain technology is an inchoate technology whose current popularity is peaking. Some of the most pervasive blockchain technology use cases exist for supply chains. Sustainable, and especially green, supply chains can benefit from blockchain technology, but there are also caveats. The sustainability and environmental management research and academic literature is only starting to investigate this emergent field. This paper seeks to help advance the discussion and motivate additional practice and research related to green supply chains and blockchain technology. This viewpoint paper provides insight into some of the main dimensions of blockchain technology, an overview of the use cases and issues, and some general research areas for further investigation.

Blockchain technology starts to reconfigure all aspects of society to make it clear and beneficial for the legal system. The chapter introduces “The Blockchain Revolution” in categories 1.0, 2.0, and 3.0; in the form of analyzing the use of the technology that is being applied in new innovative business models, Blockchain 1.0 starts with the creation of the first blockchain and the introduction of the technology in the “Bitcoin Whitepaper,” the crypto-currency model, via Bitcoin's application in services related to cash, payments, and transfers. Blockchain 2.0 starts with the indication that using smart contracts on blockchains will be available via the development of syntax (i.e., “solidity” that would enable developers to create solutions with blockchain technology at the backend). The chapter explores the feature of the new disruptive business models-based blockchain technology as a new approach in delivering business products and services. In the chapter, the authors explore the new technologies raised in different fields of business.

Analysis of resilience strategies and ripple effect in blockchain-coordinated supply chains: An agent-based simulation study

Resilience enables supply chains to reduce their proneness to disruptions and recover faster. Many existing strategies to strengthen the resilience of supply chains are facilitated by the use of digital technology. Blockchain, as one of the promising innovative technologies, enables a transparent, secure, and timely data exchange and automation via smart contracts. In this paper, we discuss the impact of blockchain technology on supply chain risk management and, in particular, on supply chain resilience. We identify potential risk-related blockchain application scenarios and examine their impact on the existing resilience strategies. We explore the impact of the most promising applications with respect to resilience by using an agent-based simulation model of a complex supply network affected by disruptions. The theoretical analysis reveals a promotion of supply chain resilience strategies, especially if smart contracts are used for risk-related collaboration. The simulation study indicates an increase in resilience if the underlying collaboration is based on time-efficient processes: The propagation of disruptions, the network recovery time, and total costs can be substantially reduced. However, depending on the duration of the disruption, negative effects can occur if process efficiency is insufficient. From our investigations, we derive insights for managers who are interested in practical implementation.

Blockchain for the IoT and industrial IoT: A review

Abstract The Internet of Things (IoT), especially the industrial IoT (IIoT), has rapidly developed and is receiving a lot of attention in academic areas and industry, but IoT privacy risks and security vulnerabilities are emerging from lack of fundamental security technology. The blockchain technique, due to its decentralization and information disclosure, was proposed as a decentralized and distributed approach to guarantee security requirements and motivate the development of the IoT and IIoT. In this paper, we first introduce the basic structure and main features of blockchain and summarize the security requirements to develop IoT and Industry 4.0. Then, we explore how blockchain can be applied to the IoT for Industry 4.0 using its security tools and technology. We describe the most relevant blockchain-based IoT applications to promote the functions and advantages of the blockchain technique on IoT and IIoT platforms. Finally, some recommendations are proposed to guide future blockchain researchers and developers.

The state of play of blockchain technology in the financial services sector: A systematic literature review

Abstract The modern trends of digitalization have completely transformed and reshaped business practices, whole businesses, and even a number of industries. Blockchain technology is believed to be the latest advancement in industries such as the financial sector, where trust is of prime significance. Blockchain technology is a decentralized and coded security system which provides the capability for new digital services and platforms to be created through this emerging technology. This research presents a systematic review of scholarly articles on blockchain technology in the financial sector. We commenced by considering 227 articles and subsequently filtered this list down to 87 articles. From this, we present a classification framework that has three dimensions: blockchain-enabled financial benefits, challenges, and functionality. This research identifies implications for future research and practice within the blockchain paradigm.

A Survey of IoT Applications in Blockchain Systems

Blockchain technology can be extensively applied in diverse services, including online micro-payments, supply chain tracking, digital forensics, health-care record sharing, and insurance payments. Extending the technology to the Internet of things (IoT), we can obtain a verifiable and traceable IoT network. Emerging research in IoT applications exploits blockchain technology to record transaction data, optimize current system performance, or construct next-generation systems, which can provide additional security, automatic transaction management, decentralized platforms, offline-to-online data verification, and so on. In this article, we conduct a systematic survey of the key components of IoT blockchain and examine a number of popular blockchain applications. In particular, we first give an architecture overview of popular IoT-blockchain systems by analyzing their network structures and protocols. Then, we discuss variant consensus protocols for IoT blockchains, and make comparisons among different consensus algorithms. Finally, we analyze the traffic model for P2P and blockchain systems and provide several metrics. We also provide a suitable traffic model for IoT-blockchain systems to illustrate network traffic distribution.

Blockchain, adoption, and financial inclusion in India: Research opportunities

Abstract The economic development of rural India requires connecting remote villages to local and global supply chains. Yet, high rates of financial exclusion inhibit rural Indians from participating in these supply networks. We review the literature on financial inclusion, adoption, and blockchain in India, and posit that to resolve financial exclusion, the four challenges of geographical access, high cost, inappropriate banking products, and financial illiteracy need to be overcome. Next, we argue that blockchain technologies hold the potential to overcome most of these challenges. However, for blockchain technologies to become the cornerstone of financial inclusion initiatives, an understanding of technology adoption in India is needed. To guide the development of such understanding, we develop a research agenda on the antecedents of adoption, adoption patterns, and outcomes of adoption. Answering these research questions will lead to a nuanced understanding of adoption of blockchain-based technologies in rural India. The practical contribution of this paper is the discussion of how blockchain can alleviate the issue of financial exclusion in rural India, thereby providing a basis for a solution that could connect rural Indians to global supply chain networks. The theoretical contribution lies in the identification of knowledge gaps that should be answered to achieve financial inclusion of rural Indians.

Blockchain: case studies in food supply chain visibility

Purpose This paper aims to investigate how blockchain has moved beyond cryptocurrencies and is being deployed to enhance visibility and trust in supply chains, their limitations and potential impact. Design/methodology/approach Qualitative analysis are undertaken via case studies drawn from food companies using semi-structured interviews. Findings Blockchain is demonstrated as an enabler of visibility in supply chains. Applications at scale are most likely for products where the end consumer is prepared to pay the premium currently required to fund the technology, e.g. baby food. Challenges remain in four areas: trust of the technology, human error and fraud at the boundaries, governance, consumer data access and willingness to pay. Research limitations/implications The paper shows that blockchain can be utilised as part of a system generating visibility and trust in supply chains. Research directs academic attention to issues that remain to be addressed. The challenges pertaining to the technology itself we believe to be generalisable; those specific to the food industry may not hold elsewhere. Practical implications From live case studies, we provide empirical evidence that blockchain provides visibility of exchanges and reliable data in fully digitised supply chains. This provides provenance and guards against counterfeit goods. However, firms will need to work to gain consumer buy-in for the technology following repeated past claims of trustworthiness. Originality/value This paper provides primary evidence from blockchain use cases “in the wild”. The exploratory case studies examine application of blockchain for supply chain visibility.

A Content-Analysis Based Literature Review in Blockchain Adoption within Food Supply Chain

According to the World Health Organization (WHO), one out of 10 people get sick from eating contaminated food. Complex food production process and globalization make food supply chain more delicate. Many technologies have been investigated in recent years to address food insecurity and achieve efficiency in dealing with food recalls. One of the most promising technologies is Blockchain, which has already been used successfully in financial aspects, such as bitcoin, and it is attracting interests from food supply chain organizations. As blockchain has characteristics, such as decentralization, security, immutability, smart contract, it is therefore expected to improve sustainable food supply chain management and food traceability. This paper applies a content-analysis based literature review in blockchain adoption within food supply chain. We propose four benefits. Blockchain can help to improve food traceability, information transparency, and recall efficiency; it can also be combined with Internet of things (IoT) to achieve better efficiency. We also propose five potential challenges, including lack of deeper understanding of blockchain, technology difficulties, raw data manipulation, difficulties of getting all stakeholders on board, and the deficiency of regulations.

Blockchain 3.0 applications survey

Abstract In this paper we survey a number of interesting applications of blockchain technology not related to cryptocurrencies. As a matter of fact, after an initial period of application to cryptocurrencies and to the financial world, blockchain technology has been successfully exploited in many other different scenarios, where its unique features allowed the definition of innovative and sometimes disruptive solutions. In particular, this paper takes into account the following application scenarios: end-to-end verifiable electronic voting, healthcare records management, identity management systems, access control systems, decentralized notary (with a focus on intellectual property protection) and supply chain management. For each of these, we firstly analyse the problem, the related requirements and the advantages the adoption of blockchain technology might bring. Then, we present a number of relevant solutions proposed in the literature both by academia and companies.

Internet of Things (IoT) is one of the recent innovations in Information Technology, which intends to interconnect the physical and digital worlds. It introduces a vision of smartness by enabling communication between objects and humans through the Internet. IoT has diverse applications in almost all sectors like Smart Health, Smart Transportation, and Smart Cities, etc. In healthcare applications, IoT eases communication between doctors and patients as the latter can be diagnosed remotely in emergency scenarios through body sensor networks and wearable sensors. However, using IoT in healthcare systems can lead to violation of the privacy of patients. Thus, security should be taken into consideration. Blockchain is one of the trending research topics nowadays and can be applied to the majority of IoT scenarios. Few major reasons for using the Blockchain in healthcare systems are its prominent features, i.e., Decentralization, Immutability, Security and Privacy, and Transparency. This paper’s main objective was to enhance the functionality of healthcare systems using emerging and innovative computer technologies like IoT and Blockchain. So, initially, a brief introduction to the basic concepts of IoT and Blockchain is provided. After this, the applicability of IoT and Blockchain in the medical sector is explored in three major areas—drug traceability, remote patient-monitoring, and medical record management. At last, the challenges of deploying IoT and Blockchain in healthcare systems are discussed.

A Survey on Zero-Knowledge Proof in Blockchain

Blockchain, which is usually regarded as a public, decentralized and distributed ledger, has attracted significant attention recently. In the environment of blockchain, all historical transaction data are recorded and stored. However, because blockchain is open and transparent, a malicious user may illegally access private transaction data, including transaction amount, account address, and account balance. As a cryptographic technique, zero-knowledge proof (ZKP) can be used to verify whether the prover has enough transaction amount in the environment of blockchain without leaking any private transaction data. This article provides a comprehensive survey on ZKP in the environment of blockchain with the aim of highlighting security problems and challenges. It first discusses the framework, models and applications of ZKP. Next, it provides an introduction of blockchain, and proposes a framework of ZKP in the environment of blockchain. Then, it highlights the current state of ZKP in the environment of blockchain. Finally, it identifies some potential problems and future research directions.

A systematic literature review of blockchain and smart contract development: Techniques, tools, and open challenges

Abstract Blockchain platforms and languages for writing smart contracts are becoming increasingly popular. However, smart contracts and blockchain applications are developed through non-standard software life-cycles, in which, for instance, delivered applications can hardly be updated or bugs resolved by releasing a new version of the software. Therefore, this systematic literature review oriented to software engineering aims at highlighting current problems and possible solutions concerning smart contracts and blockchain applications development. In this paper, we analyze 96 articles (written from 2016 to 2020) presenting solutions to tackle software engineering-specific challenges related to the development, test, and security assessment of blockchain-oriented software. In particular, we review papers (that appeared in international journals and conferences) relating to six specific topics: smart contract testing, smart contract code analysis, smart contract metrics, smart contract security, Dapp performance, and blockchain applications. Beyond the systematic review of the techniques, tools, and approaches that have been proposed in the literature to address the issues posed by the development of blockchain-based software, for each of the six aforementioned topics, we identify open challenges that require further research.

Blockchain adoption in food supply chains: a review and implementation framework

Abstract Blockchain technology has received significant attention from the food industry; however, due to the scarcity of successful Blockchain projects and sector-specific studies, a step-by-step approach for implementing Blockchain in food supply chains (FSCs) is still missing. A systematic literature review of 69 high-quality, peer-reviewed articles is utilized to capture Blockchain adoption drivers and barriers, applications, and implementation stages within FSCs. Current Blockchain issues such as scalability, regulations, privacy, and incentivization are identified as future research opportunities. Following innovation adoption theory, a three-stage conceptual framework for Blockchain implementation in FSCs is developed. The proposed framework is novel and is expected to benefit food chain managers in establishing the suitability of Blockchain for their organization and/or wider supply network. Identified influential factors, case examples, and implementation stages are expected to guide practitioners in developing a roadmap for adopting Blockchain in the food industry.

Securing federated learning with blockchain: a systematic literature review

Federated learning (FL) is a promising framework for distributed machine learning that trains models without sharing local data while protecting privacy. FL exploits the concept of collaborative learning and builds privacy-preserving models. Nevertheless, the integral features of FL are fraught with problems, such as the disclosure of private information, the unreliability of uploading model parameters to the server, the communication cost, etc. Blockchain, as a decentralized technology, is able to improve the performance of FL without requiring a centralized server and also solves the above problems. In this paper, a systematic literature review on the integration of Blockchain in federated learning was considered with the analysis of the existing FL problems that can be compensated. Through carefully screening, most relevant studies are included and research questions cover the potential security and privacy attacks in traditional federated learning that can be solved by blockchain as well as the characteristics of Blockchain-based FL. In addition, the latest Blockchain-based approaches to federated learning have been studied in-depth in terms of security and privacy, records and rewards, and verification and accountability. Furthermore, open issues related to the combination of Blockchain and FL are discussed. Finally, future research directions for the robust development of Blockchain-based FL systems are proposed.
